basis to cover aftter-hours<br />

care for a ggiv<strong>en</strong><br />

budget, but theyy<br />

are also looking intto<br />

the ways in which the<br />

provision of ccare<br />

might develop in the longer term. As a result, rather than being<br />

requested to providde<br />

a service, GPs aree<br />

giv<strong>en</strong> an inc<strong>en</strong>tive to remain in the after-hoours<br />

service schemme<br />

as contractors to the<br />

health authority.<br />

The Carson sstandards<br />

were review wed and formed the foundation<br />

for the nationnal<br />

quality requirem<strong>en</strong>nts<br />

(NQRs), which camme<br />

into effect in Januaary<br />

2005.<br />

From that timme<br />

all after-hours ser rvices had to be delivvered<br />

to these requirem<strong>en</strong>ts<br />

as a contracttual<br />

obligation, regarddless<br />

of where pati<strong>en</strong>nts<br />

lived. As part of tthis,<br />

providers hadd<br />

to regu<strong>la</strong>rly report and a exchange informaation,<br />

undertake regu<strong>la</strong>r<br />

audits, operate an appropriate comp<strong>la</strong>innts<br />

system, and demoonstrate<br />

the avai<strong>la</strong>bility<br />

of<br />

capacity to mmeet<br />

demand at all tim mes. Providers also haad<br />

to <strong>en</strong>sure that an aappropriately<br />

qualified member of staff wass<br />

avai<strong>la</strong>ble wh<strong>en</strong> needded,<br />

that they met targgets<br />

re<strong>la</strong>ting to thee<br />

initial phone call, initial<br />

consultation and fface-<br />

to-face consultattion,<br />

and that provision<br />

was made for thosee<br />

who did not speak EEnglish.<br />

The requireme<strong>en</strong>ts<br />

Social Care Act 20 008 introduced a new, single registration sysstem<br />

with CQC and mustt<br />

All adult soocial<br />

care and indeep<strong>en</strong>d<strong>en</strong>t<br />

healthcare providers p must be registered<br />

under the 20088<br />

Act (which rep<strong>la</strong>ces the Care Standards AAct<br />

Four Models usedd<br />

with Co-operatives<br />
