CHAPR I<br />

* policy governing the program and the allocation of policy-maldng<br />

responsibilities, including the administrative plans and procedures that<br />

implement the facility policy, and how the organization, policy, and<br />

program are designed for effective radiation protection<br />

* plans and procedures for radiation protection, including the document<br />

control measures employed<br />

* radiation protection training program, including the scope and content of<br />

training provided or required for all personnel, including facility operations<br />

and utilization personnel, health physics personnel, non-facility research<br />

and service personnel, security, fire, and other emergency personnel, and<br />

visitors<br />

* purpose, organization, and functions of any committees with<br />

responsibilities relating to radiation protection, including each committee's<br />

charter of responsibilities, frequency of meetings, audit and review<br />

responsibilities, scope of any audit and reviews, qualifications and<br />

requirements for committee members, and relationship to other standing or<br />

ad hoc committees for radiation protection at the facility or within the<br />

parent organization<br />

* the program for conducting facility radiation protection reviews and audits<br />

of all functional elements of the radiation protection program, including the<br />

scope of the reviews and audits, the basis for scheduling the review and<br />

audits, the qualifications of the auditors, and the process and office<br />

responsible for following up on audit findings<br />

* the system for evaluating experience from the radiation protection<br />

program, including problems and incidents so that these experiences can be<br />

used to improve facility design and the radiation protection program and to<br />

develop 'lessons learned," identify root causes, and implement effective<br />

corrective actions<br />

* the radiation protection program recordkeeping process, including record<br />

retention periods, accessibility, review, and archiving, any special review of<br />

radiation safety records for accuracy and validity, and the use of records<br />

for developing trend analyses, informing management, planning radiationrelated<br />

actions, and reporting to the regulatory authority and other duly<br />

authorized entities<br />
