Bildgebende Diagnostik: MRT des Schädels nativ sowie i.v. Kontrastmittelgabe:
Myositis des medialen Muskulus rectus in der linken Orbita.
Therapie: Absetzen von Etanercept. Unter 30 mg Prednisolon und
symptomatischer Th erapie mit Acemetacin und Paracetamol vollständiger
Rückgang der okulären Symptomatik bei weiter anhaltender Remission
der rheumatoiden Arthritis. Geplant ist zur weiteren Behandlung
der rA die Gabe von Adalimumab. In einem therapierefraktären
Fall einer idiopathischen orbitalen Myositis war Infl iximab als eff ektiv
beschrieben worden.
Diskussion: Beim Auft reten einer Orbitopathie muss nach Ausschluss
einer Infektion und einer endokrinen Orbitopathie auch an eine orbitale
Myositis gedacht werden. Eine orbitale Myositis bei rheumatoider
Arthritis wird in Kasuistiken beschrieben. In zwei Fällen trat die orbitale
Myositis unter einer Th erapie mit Etanercept auf, so dass auf einen
möglichen kausalen Zusammenhang zu achten ist.

Diff erential cytokine production of stimulated peripheral blood B
cells from patients with rheumatoid arthritis

An important role of B lymphocytes in the pathogenesis of rheumatoid
arthritis (RA) has been proposed. Th e cytokine production of B cells
may contribute to this phenomenon. Th erefore, the concentration of
diff erent cytokines and chemokines was determined in the supernatant
of activated B cells from patients with RA and healthy controls
(HC). B cells of RA patients (n=10) and HC (n=12) were isolated from
PBMC by positive selection (CD19-microbeads) using the MACS technology.
Stimulation was achieved with Staphylococcus aureus cells
strain Cowan I (SAC, 0,05%), anti-CD40 mab (1 microg/ml), and IL-4
(5 ng/ml) or soluble anti-IgM-antibodies (Fab(2) fragment, 20 microg/ml)
together with anti-CD40 and IL-4. Th e concentration of IL-2, IL-4,
IL-5, IL-6, IL-10, TNF-alpha, IFN-gamma, CXCL8/IL-8, CXCL-10/IP-
10, CCL5/RANTES, CXCL9/MIG, and CCL2/MCP-1 was determined
in the culture supernatant at 48h using cytometric bead arrays (CBA)
and sandwich ELISA. In addition, the expression of activation markers
(CD25 and CD70) was determined on B cells with fl ow cytometry. SAC
plus anti-CD40 and IL-4 induced a more potent stimulation compared
to IgM/ anti-CD40/ IL-4. Compared to HC the concentration of IL-10
and CCL5/RANTES was signifi cantly increased in the culture supernatant
of B cells from RA patients stimulated with SAC/ anti-CD40/ IL-4.
In contrast, the release of TNF-alpha, IFN-gamma, and CXCL10/IP-10
was decreased in RA patients upon stimulation with SAC/ anti-CD40/
IL-4. With regard to B cells stimulated with anti-IgM/ anti-CD40/ IL-
4 the synthesis of CCL5/RANTES was signifi cantly increased in RA
patients compared to HC. Our results demonstrate diff erences in the
production of cytokines and chemokines by stimulated B cells of RA
patients compared to HC. While the release of IL-10 and CCL5 is increased,
B cells of RA patients demonstrate an impaired synthesis of
TNF-alpha, IFN-gamma, and CXCL-10/ IP-10.

Einfl uss einer Langzeitsupplementation mit n-3 LC-PUFA auf die
Fettsäurenverteilung in den Plasmalipiden und der Erythrozytenmembran
bei Patienten mit Rheumatoider Arthritis

Da langkettige mehrfach ungesättigte n-3 Fettsäuren (n-3 LC-PUFA)
eine bedeutende Rolle im Entzündungsprozeß spielen, wird zunehmend
ihre Relevanz als therapieunterstützende Maßnahme für Patienten
mit chronisch-entzündlichen Erkrankungen diskutiert. In einer

Humaninterventionsstudie wurde untersucht, inwiefern eine Supplementation
mit n-3 LC-PUFA die Fettsäurenverteilung in den Plasmalipiden
und der Erythrozytenmembran verändert.
An der doppelblinden, placebokontrollierten Crossover-Studie nahmen
45 Patienten mit Rheumatoider Arthritis teil, die in zwei Gruppen
randomisiert wurden. Beide Gruppen erhielten die supplementierten
Milchprodukte (Verum) bzw. handelsübliche Vergleichsprodukte (Placebo)
jeweils über drei Monate. Zwischen diesen Phasen befand sich
eine zweimonatige Auswaschphase in der keine Produkte verabreicht
wurden. Die Blutabnahme erfolgte nüchtern, zu Beginn sowie am Ende
jeder Interventionssperiode. Die Milchprodukte (Joghurt, Käse und
Butter) wurden mit verschiedenen Ölen angereichert, die sich durch
hohe Gehalte an Eicosapentaensäure (EPA, C20:5n-3), Docosahexaensäure
(DHA, C22:6n-3) und a-Linolensäure (ALA, C18:3n-3) auszeichnen.
Mit diesen Milchprodukten nahmen die Probanden täglich zusätzlich
ca. 2,3 g n-3 PUFA auf (1,35 g EPA+DHA und 0,95 g ALA).
Durch den Verzehr der angereicherten Milchprodukte stieg die EPA-
Konzentration in den Plasmalipiden im Vergleich zu den Ausgangswerten
