S74 Abstracts Ergebnisse: VEGFR1 mRNA und Protein waren signifi kant höher in RA- als in OA-Gefäßen exprimiert (p=0,0433). Id2 mRNA Expression und Proteinsekretion war stärker in OA (p=0,0952). Keine signifi kante mRNA Expression <strong>für</strong> die anderen analysierten Gene konnte mit Ausnahme einer Tendenz <strong>für</strong> CD82 nachgewiesen werden, welches stärker in OA exprimiert war (p=0,099). Die Quantifi zierung der Immunhistologie im Lining zeigte, dass Id2 und CD82 verstärkt im RA-Lining vorkommen (p=0,0079 und 0,0317). Bezüglich der medikamentösen Behandlung der Patienten, wurde eine erhöhte VEGF-Expression bei RA-Patienten, die oral Prednisolon erhalten hatten im Vergleich zu RA-Patienten ohne Prednisolon, festgestellt. Diskussion: Die Experimente zeigen, dass mittels LMM selektiv die perivaskuläre Genexpression von Angiogenese-regulierenden Molekülen darstellbar ist. VEGF, ein wichtiger Faktor <strong>für</strong> Angiogenese, weist eine deutliche Relation zur RA-Pathogenese auf. Die erhöhte VEGFR-1 Expression in synovialen RA-Gefäßen verdeutlicht die gesteigerte Angiogenese. Im Gegensatz dazu ist Id2, welches durch Hypoxie im RA-Synovium hochreguliert ist, geringer in RA Mikrogefäßen exprimiert und konnte vermehrt im RA-Lining gemessen werden. Dies bestärkt das Konzept, dass Id2 im RA-Synovium nur in hypoxischen Bereichen der Invasion, aber nicht peripher eine pathophysiologische Rolle spielt. Recent investigations demonstrated regulatory CD4+T cells in synovial fl uid (SF). Impaired function of regulatory T cells has been discussed, but to be eff ective Treg need to mediate their anti-infl ammatory activity in the synovial tissue (ST). Objectives: To search for the presence and function of Tregs in RA- ST, SF, and autologous peripheral blood (PB). To detect IFN-γ secretion and their modulation in primary synovial tissue cell (STC) cultures by co-culture with autologous PB CD4+T cells. To analyse the distribution of T-bet+, respectively IFN-γ-producing lymphocytes and Foxp3+, respectively regulatory T cells at transcription level in ST, PB and SF. Methods: Samples of 51 RA patients were included. ST were obtained by surgery and digested with collagenase. STC were incubated in anti- IFN-γ or anti-IL-10 coated Elispot-plates. Experiments were modifi ed by depletion of ST-T cells using anti-CD3 magnetic beads, and by adding autologous PB CD4+. IFN-γ and IL-10 frequency was examined by ELISPOT. Mononuclear cells (MC) from SF and PB were isolated by Ficoll density gradient centrifugation. Total RNA of ST, PB, SF was isolated. Real-time RT-PCR were performed by TaqMan® Gene Expression Assays for Tbet, Foxp3, CD3epsilon and 18S rRNA. Tregs in ST were identifi ed by immunohistochemistry for Foxp3. Results: Frequency (FR; FR data mean throughout FR/10000 cultured cells) of IFN-γ secreting cells in RA-ST was 64.6 ± 20.0(n=13; mean ± SEM). Non-stimulated IFN-γ FR of osteoarthritis tissue cells (OA- STC) was determined to be 7.0 ± 2.5(n=5) being signifi cantly diff erent (P=0.03). Mean basal IL-10 FR was 218.0 ± 94.0(n=5) in complete RA- STC and 161.0 ± 86.0(n=4) from OA-STC; not signifi cantly diff erent (P=0.9). CD3 depletion almost completely abrogated IFN-γ production of RA-STC (mean FR 0.67 ± 0.55(n=3; P=0.01). PB CD4+ cells were able to signifi cantly reduce the IFN-γ FR of STC cells. Th e mean basal IFN-γ FR of PB CD4+ was 7.8 ± 3.4,(n=13) and the mean basal IFN-γ FR of STC was 64.6 ± 20.0(n=13). Th e co-culture of these STC with autologous PB-CD4+ led to a reduced IFN-γ FR of 37.6 ± 17.4 (n=13) signifi cantly (P = 0.002), while IL 10 was not signifi cantly | <strong>Zeitschrift</strong> <strong>für</strong> <strong>Rheumatologie</strong> · <strong>Supplement</strong> 1 · 2006 changed by co-culture of STC and PB CD4+. As compared to PB and SF, a high ratio of Tbet/Foxp3 was detectable in ST exclusively, which correlated with a higher CD3epsilon mRNA, and a higher clinical disease activity score. Conclusion: Th ese experiments show that PB CD4+ T cells with regulatory capacity of RA patients are potent inhibitors of IFN-gamma-expression in RA-ST. In addition these data suggest a selective distribution in RA. If these fi ndings can be further confi rmed they may elucidate, why despite enhanced number of regulatory T cells in SF suppression of infl ammation is insuffi cient. Klinik II, JLU Gießen Hintergrund: Adipokine wie das Adiponektin beeinfl ussen neben dem Energie-stoff wechsel auch Entzündungsreaktionen der Rheumatoiden Arthritis (RA). Re-sistin zählt zur Familie der Cystein-reichen Proteine, akkumuliert in entzündeten Gelenken von RA-Patienten und bewirkt bei intraartikulärer Resistininjektion Synovitis mit Knorpelzerstörung im Mausmodell. Surfactant Protein A (SP-A), ein Mitglied der Collectin-Familie, wird als wesentlicher Bestandteil des Surfactant v.a. in der Lunge exprimiert. Patienten mit RA weisen erhöhte SP-A Konzentrationen in der Synovialfl üssigkeit und SP-A spezifi sche Autoantikörper auf. SP-A besitzt auff ällige Homologien zu Adiponektin und C1q. Die Funktion von Resistin und SP-A in der RA ist unklar, beeinfl ussen möglicherweise jedoch Entzündungsreaktionen und Knorpelzerstörung. Daher wurden die Auswirkungen von Resistin, SP-A und Adiponektin auf Entzündungsfaktoren und Matrix-abbauenden Enzymen in synovialen Fibroblasten(SF)untersucht. Methoden: RASF wurden aus synovialen Biopsien von RA- und OA- Patienten isoliert und unter Standardbedingungen bis zur Passage 4 kultiviert. Die Zellen wurden bei einer Konfl uenz von 70% mit unterschiedlichen Konzentrationen an Resistin, SP-A und Adiponektin stimuliert, um deren Einfl uss auf die Expression von Zytokinen und MMPs mittels ELISA zu untersuchen. Ergebnisse: Resistin induziert die Expression von IL-6 und IL-8 in OASF (1.7-, 5.2-fach), in RASF nur gering und nicht signifi kant (1.1-, 1.4-fach). Adiponektin induzierte diese Zytokine ebenfalls (8.3-, 11-fach in RASF). Resistin reduzierte die Expression von Activin in RASF (2.5-fach), hatte aber keinen Eff ekt in OASF. Im Gegensatz zum Adiponektin, dass die TIMP-1-Expression in RASF induziert (2-fach), hatte Resistin keinen Eff ekt. Adiponektin induziert die Sekretion von MMP-3 und proMMP-1 in RASF (10-, 8.2-fach), Resistin zeigte keine Wirkung auf die Expression von MMP-3, TNF-alpha und IL-12. Eine Stimulierung mit SP-A zeigte keine signifi kanten Änderungen. Zusammenfassung: Die Daten zeigen, dass sowohl Resistin als auch Adiponektin, nicht aber SP-A, die Entzündungsreaktion der RA beeinfl ussen, indem sie die Bildung von proinfl ammatorischen Zytokinen und MMPs in SF induzieren.
