Bei unseren Patienten kam es unter der Behandlung zu keinen schwerwiegenden<br />
Nebenwirkungen. Ein Patient verstarb während des Nachbeobachtungszeitraumes,<br />
ein kausaler Zusammenhang zur Rituximab<br />
Behandlung bestand nicht.<br />
Fünf Patienten zeigten eine Verbesserung der Krankheitsaktivität im<br />
ECLAM um mindestens zwei Punkte nach sechs Monaten, darunter<br />
zwei Patienten mit Nierenbeteiligung, eine Patientin mit Autoimmunthrombozytopenie<br />
und ein Patient mit führender Hautbeteiligung.<br />
Die drei Patienten mit ZNS-Beteiligung zeigten einen anhaltenden Remissionserhalt<br />
über die Dauer der Nachbeobachtung, zur Zeit 6, 8 und<br />
12 Monate.<br />
Die selektive B-Zell Depletion in der Behandlung des systemischen Lupus<br />
erythematodes scheint eine sichere und eff ektive Behandlungsoption<br />
bei verschiedenen Krankheitsausprägungen darzustellen.<br />

Background: Lupus nephritis is considered to be mainly mediated by<br />
deposition of autoantibodies and immuncomplexes (IC) in the glomeruli.<br />
However several lines of evidence indicate that autoantibodies/IC<br />
are not the sole pathogenic factor for the occurrence of lupus nephritis.<br />
Kidney infi ltrating leukocytes, consisting of mainly CD4+ T cells, probably<br />
are potent contributors to the local damage and might play a key<br />
role in the pathogenesis of the nephritis.<br />
Objective: To analyse the infi ltrating CD4+ T cells in terms of general<br />
T cell characteristics and to identify crucial mechanisms of cell recruitment<br />
into the kidneys.<br />
Methods: We isolated the infi ltrating T cells from nephritic<br />
NZB/W F1 mice by fi rst washing the blood out of the kidneys to avoid<br />
contamination with PBMC´s and then separating the infi ltrating leukocytes<br />
from other renal cells by density centrifugation. We analysed<br />
the surface expression of CD62L, CD44, CD45RB, CD69 and CXCR3<br />
on the infi ltrating CD4+ T cells and determined their cytokine production<br />
aft er stimulation with PMA/Ionomycin. Furthermore we assessed<br />
the migratory capacity of the isolated cells towards CXCL10 in trans-<br />
well migration assays and determined the source of this chemokine in<br />
the kidneys by immunohistochemistry.<br />
Results: We analysed the surface expression of CD62L, CD44 and<br />
CD45RB on the isolated CD4+ T cells and determined that 55-60% of<br />
the infi ltrating T cells were of the memory/eff ector phenotype while<br />
40-45% were naive T cells. Almost 30% of the CD4+ T cells expressed<br />
CD69, refl ecting recent activation of the cells. Th e infi ltrating CD4+ T<br />
cells predominantly produced infl ammatory cytokines upon stimulation<br />
with PMA/Ionomycin (16.8% produced IFNgamma, 11.8% TNFalpha),<br />
whereas only few cells producing IL-4 (1.1%) or IL-10 (5.8%)<br />
were detectable. 30-50% of CD4+ cells expressed the Th 1 associated<br />
chemokine receptor CXCR3. In transwell migration assays the majority<br />
(70%) of memory/eff ector T cells migrated towards CXCL10 in a<br />
dose dependent manner, whereas the kidney infi ltrating naive T cells<br />
did not migrate towards CXCL10. By immunofl uorescence staining of<br />
kidney sections we were able to identify CXCL10 producing cells with a<br />
dendritic cell like phenotype in the kidney interstitium.<br />
Conclusion: CXCR3 positive Th 1 eff ector T cells are recruited into the<br />
infl amed kidney tissue at least partly via CXCL10. Th ese cells show signs<br />
of recent cell activation and might represent a key subset in the<br />
pathogenesis of lupus nephritis, probably by mediating local damage<br />
via the production of proinfl ammatory cytokines.<br />

Das Vaskulitis-Register Schleswig-Holstein: Stabile Inzidenzraten von<br />
1998 bis 2005<br />

Ziel der Studie: Erfassung aller neu diagnostizierten primär systemischen<br />
Vaskulitiden (PSV) in Schleswig-Holstein zur Bestimmung<br />
der jährlichen Inzidenzraten <strong>für</strong> PSV von 1998 bis 2005.<br />
Methoden: Vom 01.01.98 bis 31.12.2005 wurden alle Fälle neu diagnostizierter<br />
PSV in Schleswig-Holstein, einer Region mit 2.823.171 Ein -<br />
wohnern registriert. Patienten wurden gemeldet von (a) allen Krankenhausabteilungen<br />
und Polikliniken (b) allen pathologischen Abteilungen<br />
und (c) den immunologischen Referenzlaboren der Region.<br />
Ergebnisse: Im Zeitraum von 1998 bis 2005 wurden über acht Jahre<br />
965 Patienten mit PSV gemeldet. Die Inzidenz-Raten <strong>für</strong> alle PSV variieren<br />
zwischen 38 (2004) und 54 (1998)/1 Million Einwohner/ Jahr. Die<br />
häufi gste PSV ist die Riesenzellarteriitis mit Inzidenzraten zwischen 25<br />
und 46 in der Population über 50 Jahre. Inzidenzraten der ANCA assoziierten<br />
PSV (Wegener’sche Granulomatose (WG), mikroskopische<br />
Polyangiitis (MPA), Churg-Strauss Syndrom (CSS)) lagen zwischen<br />
9.5 und 16/1 Million Einwohner/Jahr. Die WG wurde zwei bis dreimal<br />
häufi ger als die MPA und CSS beobachtet. Kutane leukozytoklastische<br />
Vaskulitiden wurden 2004 und 2005 seltener beobachtet. Die Ergebnisse<br />
sind stabil über den Zeitraum von acht Jahren.<br />
Schlussfolgerung: Die Ergebnisse des Vaskulitis-Registers Schleswig-<br />
Holstein zeigen stabile Inzidenzraten <strong>für</strong> alle PSV von 1998 bis 2005<br />
in einer Bevölkerung mit ca. 2.8 Millionen Einwohnern. Die Zahlen<br />
sind vergleichbar mit anderen Europäischen Untersuchungen kleiner<br />
Regionen oder Vaskulitis-Zentren. Am Beispiel der WG liegen die Inzidenzraten<br />
<strong>für</strong> die WG in Schleswig-Holstein zwischen 6-12 (im Mittel<br />
bei 8.5)/1 Million Einwohner, in Norwegen bei 8/1 Million Einwohner,<br />
in Großbritannien bei 10.6/1 Million Einwohner und wurden in Spanien<br />
mit 2.95/1 Million Einwohner etwas niedriger angegeben.<br />
