Die individuellen Verläufe der Höhe der CCP-AK korrelierten nicht<br />
mit dem DAS28.<br />
Diskussion: Die Höhe der CCP-AK korreliert schwach aber signifi -<br />
kant mit der Krankheitsaktivität (DAS28), der Korrelationskoeffi zient<br />
und das Signifi kanzniveau ist den bekannten Parametern (BKS, CRP,<br />
RF) jedoch deutlich unterlegen. Auch im individuellen Verlauf ist der<br />
CCP-AK nicht als Parameter zur Einschätzung des Krankheitsverlaufs<br />
zu verwenden.<br />
Eine regelmäßige Bestimmung der CCP-AK bei bekannter RF/CCP-<br />
AK positiver rheumatoider Arthritis erscheint daher nicht sinnvoll.<br />
Einfach zu bestimmende Parameter (MS, HAQ) bzw. kostengünstig<br />
zu bestimmende Parameter (BKS) sind wesentlich besser zur Abschätzung<br />
der Krankheitsaktivität geeignet.<br />

with the HLA-DRB1 shared epitope and rheumatoid arthritis<br />

Purpose: It can be suspected that inherited polymorphisms at codons<br />
16, 27 and 164 of the beta2-adrenergic receptor (beta2R) gene as well<br />
as HLA alleles may turn out to be important in the development and<br />
maintenance of rheumatoid arthritis (RA). Th erefore, the frequency of<br />
the main polymorphisms in the beta2R gene and the association with<br />
HLA-DRB1 alleles and RA was examined.<br />
Methods: An allele-specifi c polymerase chain reaction was used to determine<br />
the common variants of the beta2R at position 16, 27, and 164<br />
in patients with RA (n= 310) and ethnically matched healthy controls<br />
(n= 305) from Germany. In a subgroup of RA patients and controls<br />
HLA-DRB1 genotyping was performed by oligonucleotide hybridisation<br />
of enzymatically amplifi ed DNA allowing low-resolution HLA-<br />
DRB1 genotyping comprising specifi cities DRB1*01 to DRB1*17 and<br />
the identifi cation of alleles pertinent to the assessment of the shared<br />
epitope and the DERAA-positive DRB1 alleles.<br />
Results: Th e age of the RA patients was 47.2 ± 9.8 years with disease<br />
duration of 18.0 ± 3.4 years. 271 RA patients (87.4 %) were positive for<br />
the rheumatoid factor. Th e distribution of the beta2R alleles in controls<br />
and RA patients followed Mendelian inheritance. Arginine (Arg)<br />
at codon 16 was present in 278 RA patients (89.7 %) compared to 202<br />
controls (66.2%; OR 4.43, 95 % CI 2.81 to 7.02, p=0.00001). Homozygosity<br />
for Arg16 was found in 107 RA patients (34.5 %) compared to 14<br />
controls (4.6 %; OR 10.9, CI 5.9 to 20.5; p=0.00001). Homozygosity for<br />
Glutamine (Gln) at codon 27 was present in 94 RA patients (30.3%)<br />
compared to 67 controls (22%; OR 1.5, 95% CI 1.06-2.26, p=0.023).<br />
Interestingly, the patients with the Arg16 allele have a younger age of<br />
