1.4 Obtain and Analyze Initial Objective Evidence<br />

Purpose<br />

Obtain in<strong>for</strong>mation that facilitates site-specific preparation and an<br />

understanding of the implementation of model practices across the<br />

organizational unit. Identify potential issues, gaps, or risks to aid in refining<br />

the plan. Strengthen understanding of the organization’s operations and<br />

processes.<br />

Entry Criteria • <strong>Appraisal</strong> input received<br />

• Sponsor authorization to proceed<br />

• Availability of practice implementation data <strong>for</strong> organizational unit<br />

Inputs • Practice implementation data <strong>for</strong> organizational unit<br />

• Identified participants<br />

• The team is ready to make detailed plans <strong>for</strong> data collection.<br />

Exit Criteria • All objective evidence captured during this activity has been recorded <strong>for</strong><br />

later use.<br />

• High-priority areas <strong>for</strong> additional data collection have been identified.<br />

• The level of sufficiency of the objective evidence to support the appraisal<br />

is determined.<br />
