<strong>Appraisal</strong> Disclosure<br />

Statement<br />

The <strong>Appraisal</strong> Disclosure Statement (ADS) provides in<strong>for</strong>mation considered essential to adequately<br />

interpret the meaning of maturity level or capability level ratings resulting from a<br />

<strong>CMMI</strong> Class A appraisal.<br />

The ADS is prepared by the appraisal team leader and provided to the appraisal sponsor at the<br />

conclusion of the appraisal. If the final findings briefing reports the appraisal ratings, the vehicle<br />

<strong>for</strong> reporting the ratings must be the ADS. Otherwise the appraisal team leader delivers<br />

the ADS to the sponsor as a separate document.<br />

ADS Content<br />

The ADS consists of the following in<strong>for</strong>mation:<br />

• identification of appraisal sponsor and sponsor’s organizational affiliation<br />

• identification of appraisal team leader and appraisal team members and their organizational<br />

affiliations<br />

• identification of organizational unit appraised (the unit to which the ratings are applicable<br />

and the domains examined, as defined in the appraisal plan)<br />

• identification of <strong>CMMI</strong> model (version, representation, and domains)<br />

• identification of appraisal method (name and version)<br />

• itemization of process areas rated and process areas not rated<br />

• maturity level and/or capability level ratings assigned<br />

• dates of on-site activity<br />

• date of issuance of ADS<br />

• statement affirming that all <strong>SCAMPI</strong> requirements were met<br />

• signature of appraisal team leader (at a minimum); those of appraisal team members and<br />

appraisal sponsor are optional<br />
