OHM RemediationServices Corp. HEALTH AND SAFETY PLAN12. Excavations <strong>and</strong> Trenching - Excavations <strong>and</strong> trenching present a special risk to<strong>work</strong>ers due to the hazard of trench wall collapse. If OHM personnel must enterexcavations five feet or greater in depth, the sides of the excavation will be sloped l-l/2:1 (horizontal:verticaI) or shored in accordance with 29 CPR 1926.650 through652.All surface encumbrances will be located <strong>and</strong> those that may create a hazard <strong>and</strong>remove or support. All underground installations, utilities, sewer, telephone, fuel,electric, <strong>and</strong> water lines will be located <strong>and</strong> marked prior to opening excavation.Structural ramps <strong>for</strong> equipment or personnel must be designed by a qualifiedindividual. Employees will not be permitted underneath loads h<strong>and</strong>led by liftingequipment.A warning system such as barricades, h<strong>and</strong> or mechanical signals, or stop logs shallbe used to warn mobile equipment operators does not have a clear view of theexcavation’s edge. Excavations which have a potential <strong>for</strong> hazardous atmospheremust be tested be<strong>for</strong>e entry. Excavations which have hazardous atmospheres musthave emergency rescue equipment present be<strong>for</strong>e entry. Precautions will be taken toprotect employees from water accumulation. Support systems will be provided <strong>for</strong>buildings, walls, or other structures when their stability is affected by the excavation.All materials must be kept two feet from the excavation’s edge to protect employeesfrom falling objects. Daily inspection of the excavation by a qualified individual shahbe conducted. The excavation <strong>soil</strong> type will be determined <strong>and</strong> employee protectionsystems will be used. The OHM excavation permit will be completed by the OHMcompetent person when employee exposure can reasonably be anticipated. This isrequired to document compliance with 29 CPR 1926 Subpart P - Excavations. Theexcavation permit is required to document inspections by the competent person daily,be<strong>for</strong>e <strong>work</strong>, <strong>and</strong> as needed throughout the shift13. Pumninz Eauinment - Various types of pumps may be used <strong>for</strong> the removal ofmaterials from ditches, ponds, lagoons, etc. The h<strong>and</strong>ling of pressurized hoses thatcould rupture <strong>and</strong> violently release liquid materials to the <strong>work</strong> area will be controlledby inspecting all hose fittings <strong>for</strong> secure connections [all OPW (camlock) <strong>and</strong> fittingsmust be secured with the wire]. All employees must wear splash gear includingsplash shields when moving or disconnecting pumps <strong>and</strong> hoses.14. Drum H<strong>and</strong>linp <strong>and</strong> Opening - No unknown drums are anticipated.15. Head <strong>and</strong> Back Iniuries - As minimum requirements, hard hats <strong>and</strong> safety glasseswill be donned prior to per<strong>for</strong>ming any <strong>site</strong> activities.
