OHM RemediationServices Corp. SAMPLING AND ANALYSIS PLANlegibly written. The language will be factual <strong>and</strong> objective. No erasures will be permitted. Ifan incorrect entry is made, the error will be crossed out with a single strike mark, initialed, <strong>and</strong>dated. When audits are per<strong>for</strong>med, the auditor’s remarks <strong>and</strong> decisions must also appear inthese notes. These audits should be followed up by written report submitted by the auditor,including opinions <strong>and</strong> conclusions. A copy of this report should be placed in the project file<strong>and</strong> one copy kept in the <strong>sampling</strong> file <strong>for</strong> easy reference.This in<strong>for</strong>mation will also be entered into the database program that has been prepared <strong>for</strong> this<strong>site</strong>. It will be entered daily by the field chemist or sample tech. This person will be the pointof contact <strong>for</strong> all <strong>sampling</strong> <strong>and</strong> analytical in<strong>for</strong>mation. Report outputs from the database areacceptable substitutes <strong>for</strong> the sample log book3.8 Sample LabelsAny samples placed into a sample container will be identified by a sample label. Jncluded onthe label are the following in<strong>for</strong>mation:1) JOB NUMBER2) DATE -- Month, day, year3) TIME-Militarytime4) SAMPLE NUMBER -- see section 3.2 <strong>for</strong> designations5) SAMPLE DESCRIPTION6) SAMPLER -- Sampler’s name7) PRESERVATIVES8) ANALYSIS REQUESTED -- see Appendix A, Table A-lThe in<strong>for</strong>mation described above will be printed neatly using an indelible marker. After thesample is taken <strong>and</strong> the label is securely attached, the sample is logged into the sample logbook. An example of a sample label is included in Appendix B.3.9 Custody SealsCustody seals are narrow strips of adhesive tape of glass fiber used to demonstrate that notampering has occurred. They may be used on <strong>sampling</strong> equipment, sample transportcontainers, <strong>and</strong> individual sample jars. They will be signed <strong>and</strong> dated by the sampler <strong>and</strong>placed from one side, across the top, <strong>and</strong> to the other side of the sample bottle or across theopenings of the sample transport containers.
