xiiw OHM RemediationServices Corp.3.7 Sample Log BookSAMPLING AND ANALYSXS PLANIt is necessary <strong>for</strong> the <strong>sampling</strong> crew to maintain daily field notes. Items included are <strong>sampling</strong>protocol, any changes to the procedures, meetings, instructions, safety precautions, personnelprotection, <strong>and</strong> activities pertaining to the samples. The person taking notes will beknowledgeable about these activities <strong>and</strong> record pertinent details.Repetition of in<strong>for</strong>mation recorded in other permanent logs should be avoided, but enoughshould be recorded to present a clear <strong>and</strong> accurate picture of technical activities. At a later date,should a question arise concerning a specific event or a procedure used, it will be answeredfrom these notes. The following in<strong>for</strong>mation should be logged into the logbooks <strong>and</strong>/ordatabase:...Date <strong>and</strong> time of <strong>sampling</strong>Sample number, locations, type, matrices, volumes, sample ID <strong>and</strong> descriptions, type<strong>and</strong> number of sample containers, names <strong>and</strong> signatures of individuals per<strong>for</strong>ming<strong>sampling</strong> tasks, COC <strong>and</strong> airbill numbers, preservatives, <strong>and</strong> date samples were sentName of laboratories <strong>and</strong> contacts to which the samples were sent, TAT requested, <strong>and</strong>data results, when possible.Terminationof a sample point or parameter <strong>and</strong> reasons....Unusual appearance or odor of a sampleMeasurements, volume of flow, temperature, <strong>and</strong> weather conditionsAdditional samples <strong>and</strong> reasons <strong>for</strong> obtaining themLevels of protection used (with justification)...Meetings <strong>and</strong> telephone conversations held with LANTDIV,agencies, project manager, or supervisor.Details concerning any samples split with another partyDetails of QC samples obtainedNTR, regulatoryThese notes will be dated <strong>and</strong> signed (each page) <strong>for</strong> validity in a court of law.
