406Chapter 18 – Summary of Proposals for ImplementationDrug and Therapeutics Formulary CommitteeFormulary CommitteeThe Formulary Committee is established under the name Drug andTherapeutics Formulary Committee in English and Comité duformulaire des médicaments et des thérapeutiques in French.CompositionThe Formulary Committee shall be composed of such number ofmembers as may be appointed by the Lieutenant Governor inCouncil and shall consist of:(a) the Chair of the Agency, ex officio;(b) equal representation from the College of Physicians andSurgeons of Ontario, and the Ontario College ofPharmacists;(c) an expert in pharmacology or pharmacotherapy who maybe an educator,(d) one member of a College that is not already representedunder subsection (b);(e) a representative of the Ontario Drug Benefits Program ofthe Ontario Ministry of Health and Long-Term Care.The Lieutenant Governor in Council shall appoint therepresentatives listed in subsections (b) (c) (d) and (e) on theAgency’s recommendation.ChairThe Chair of the Agency shall serve as the chair of the FormularyCommittee.Terms of membersThe members listed in subsections (b) (c) and (d) shall be appointedfor a fixed period not to exceed three years. Members of theFormulary Committee are eligible for reappointment. Upon expiry oftheir term, members of the Formulary Committee shall remain inoffice until reappointed or replaced.Replacement membersA person appointed to replace a member of the Formulary Committeebefore the member’s term expires shall hold office for the remainderof the term.HPRAC Critical Links January 2009
