- 주의점 : 생명을 위협할 정도의 고혈압 발작이 발생될 수 있어
가급적 피하는 것이 좋다(응급상황을 대비하여 phentolamine 10
mg을 준비).
- 공복 상태에서 30분간 침상 안정 후 기저 혈압과 기저 혈장 NE,
E을 측정하고, 글루카곤 1.0 mg을 정맥 주사한 후 2분 후에 혈장
NE, E 및 1분 간격으로 10분간 혈압을 측정한다.
- 판정 : 혈장 카테콜라민이 2000 pg/mL 이상으로 증가하거나, 기
저치의 3배 이상 증가하면 양성
마) 클로니딘 억제 검사
- 적응증 : 혈압이 160/90 mmHg 이상이며 혈장 카테콜라민이
1000-2000 pg/mL인 경우에 시행
- 0.3 mg 클로니딘을 복용시킨 후 3시간 후에 혈장 NE을 측정
- 판정 : 500 pg/mL 또는 기저치의 50% 이하로 감소하지 않을 경우 양성
바) 방사선학적 방법
- MRI, CT, MIBG (metaiodobenzylguanidine) scan, angiography, PET
Evaluation and treatment of catecholamine-secreting tumors. Clinical suspicion is triggered by the following:
paroxysmal symptoms (especially hypertension); hypertension that is intermittent, unusually labile, or resistant to
treatment; fami ly histor y of phe ochromocytoma or associate d conditions; or incidentally discovered adrenal mass.
