T 4.3 Inoperability of existing safeguards<br />

Due to technical defects or external factors (e.g. on account of ageing,<br />

operating errors, deficient maintenance, manipulation, power failure), safety<br />

devices may become inoperative, resulting in their protective effect being<br />

greatly reduced or neutralised altogether. Examples include:<br />

- defective door locks;<br />

- inadequately functioning fire extinguishers;<br />

- soiled fire detection devices;<br />

- damaged keys or ID cards;<br />

- jamming bolt contacts on doors;<br />

- burn-in of static pictures (freezing frames) in control monitors;<br />

- wedging of fire exit doors.<br />
