faire partie des demandes des millennials ?<br />
H.C : Oui nous sommes aujourd’hui confrontés aux générations<br />
X, Y et même Z. Ces produits ne seront certes pas réservés à tel<br />
ou tel type de patientèle mais ils peuvent devenir par exemple<br />
une bonne indication pour corriger des asymétries faciales qui<br />
peuvent apparaitre très jeune. Attention toutefois aux effets<br />
à long terme et de ne pas se faire injecter trop tôt au risque<br />
d’avoir une peau d’un aspect un peu « plastique » car l’acide<br />
hyaluronique, s’il disparaît avec le temps, stimule le collagène<br />
par les fibroblastes. Des peaux de patients injectés avec une<br />
rhéologie très puissante il y a 20 ans ont quelquefois un aspect<br />
siliconé qui avait disparu. Ces répercussions seront à surveiller<br />
de près, voir à freiner. Nous aurons à choisir entre des produits<br />
très hydratants ou des produits très repulpants selon les âges.<br />
Le retour de produits type Macrolane ?<br />
H.C : Peut-être mais sous une forme un peu différente. On a<br />
quand même beaucoup progressé en 10 ans sur la rhéologie et<br />
c’est cela qui reste très intéressant.<br />
Quelles sont les autres innovations à venir ?<br />
H.C : Il y en a beaucoup. Prenons un premier exemple avec<br />
l’échographie. Il existe aujourd’hui des appareils sophistiqués<br />
qui vont nous aider dans notre démarche sécurité pour voir<br />
s’il n’y a pas un kyste, une infection latente ou un produit qui<br />
aurait déjà été injecté en profondeur (on est surpris de se rendre<br />
compte qu’il y a des acides hyaluroniques qui restent en place<br />
beaucoup plus longtemps que prévu avec des patientes ne se<br />
souvenant plus d’avoir été injectées), ce qui n’est pas forcément<br />
facile à voir, de vérifier s’il n’y pas de ganglions, etc… En moins<br />
de 5 minutes vous pouvez repérer les artères et savoir à quelle<br />
profondeur se trouvent les vaisseaux, s’il existe une différence<br />
de volume à droite ou à gauche du visage… On peut même<br />
évaluer quels sont les bons volumes de fillers à mettre.<br />
Réveiller les produits qui sont en nous !<br />
Prenons un second exemple avec le PRP, les cellules souches ou<br />
encore les peptides qui commencent à apparaitre sur le marché.<br />
En fait, le but ici est de réveiller des produits qui étaient déjà en<br />
nous mais qui ont été « oubliés » par notre organisme. Attention<br />
à disposer des bons récepteurs, car sur certains patients les<br />
résultats seront là alors que sur d’autres cela ne marchera pas.<br />
Nous sommes aux prémices de produits qui réveillent un corps<br />
endormi. On ne cherche donc pas à ajouter du volume mais à<br />
stimuler notre organisme.<br />

These products have one major benefit: we can make them<br />
disappear if we want to, namely using hyaluronidase. Did you<br />
know that, in the United States, there are now doctors who<br />
specialise in “deflating” patients with hypertrophy?<br />
Does this demand for more volume mainly<br />
come from Millennials?<br />
H.C.: Yes, today we are coming face to face with generations X, Y<br />
and even Z. These products are not only for one particular type<br />
of patient, and they can be useful for correcting facial asymmetry,<br />
which can appear at a very young age. However, it is important<br />
to pay attention to the long term effects and not to inject too<br />
young, as you run the risk of making the skin look a bit “plastic”<br />
since, though hyaluronic acid fades away over time, it stimulates<br />
collagen via the fibroblasts. The skin of patients who have been<br />
injected with a very high rheology 20 years previously sometimes<br />
has a silicone-looking appearance, which had been phased out.<br />
These repercussions need watching closely or stopping. We have<br />
to choose between very hydrating products or very plumping<br />
products, depending on the patient’s age.<br />
Does it herald the return of products like<br />
Macrolane?<br />
H.C.: Maybe, but in a slightly different form. We have made a lot<br />
of progress in terms of rheology over the last 10 years, and that’s<br />
what is so interesting.<br />
What other innovations are in the pipeline?<br />
H.C.: There are plenty. Let’s use ultrasound as our first example.<br />
Today, there are some very sophisticated devices that help us in<br />
terms of safety, to make sure there are no cysts, latent infections<br />
or products that have already been injected in depth (you’d be<br />
surprised to hear that some hyaluronic acids last much longer<br />
than expected, so much so that patients forget they’d ever had<br />
the injections), which are not always easy to see, check there is<br />
no swelling, etc. In less than 5 minutes, you can locate the arteries<br />
and find out how deep the vessels are, if there is a difference in<br />
volume between the left and right hand side of the face… We can<br />
even assess the right volume of filler to inject.<br />
Reviving products that are already inside us!<br />
Our second example might be PRP, stem cells or even peptides,<br />
which are starting to appear on the market. In fact, the aim here<br />
is to awaken products that were already inside us but which our<br />
bodies have “forgotten about”. Make sure you have the right<br />
receptors, as in some patients the results will be visible but in<br />
others it won’t work.<br />
We are in the very early stages of products that awaken a dormant<br />
body. Here, we are not looking to add volume but rather to<br />
stimulate the body.<br />
