3. Key Personnel<br />
especially qualified <strong>for</strong> the <strong>design</strong> of infrastructure projects. The proposer shall<br />
provide such in<strong>for</strong>mation in response to Question #10 of Form 255.<br />
For each title of Key Personnel listed in Attachment 4, the proposer shall identify the<br />
individuals it will provide, throughout the term of the contract, to per<strong>for</strong>m the required<br />
<strong>services</strong>. Such individuals must be employees of the proposer or its subconsultant.<br />
The proposer shall identify multiple individuals <strong>for</strong> each title; provided, however, it shall<br />
only identify those individuals it or its subconsultant(s) has the ability to provide.<br />
For all proposed Key Personnel, the proposer must submit the individual's resume <strong>and</strong><br />
any other in<strong>for</strong>mation detailing his/her number of years of experience, as well as<br />
technical <strong>and</strong> professional qualifications. Any proposed Key Personnel provided by the<br />
Consultant <strong>and</strong>/or Subconsultant must satisfy the minimum <strong>requirements</strong> per title set<br />
<strong>for</strong>th in Exhibit D to the attached Contract. All personnel per<strong>for</strong>ming <strong>services</strong> <strong>for</strong> any<br />
Project(s) assigned to the Consultant must be approved in advance by the<br />
Commissioner.<br />
4. Minimum Requirements <strong>for</strong> Key Personnel:<br />
Below are the minimum <strong>requirements</strong> per title <strong>for</strong> the Key Personnel required <strong>for</strong> the<br />
duration of this <strong>requirements</strong> contract. The Proposal will be rejected as nonresponsive<br />
if the proposer fails to identify Key Personnel who meet the minimum<br />
<strong>requirements</strong> per title.<br />

(3) The minimum <strong>requirements</strong> <strong>for</strong> the specified titles shall be the <strong>requirements</strong><br />

Engineers (ASCE). The applicable <strong>requirements</strong> <strong>for</strong> the title in question shall be<br />
the most current <strong>requirements</strong> promulgated by the ASCE <strong>for</strong> that title as of the<br />
date on which the Consultant submitted its Proposal <strong>for</strong> the Contract.<br />
(4) If a title requires a professional license, such license must be issued by the State<br />
of N.Y.<br />
