Synonyms <strong>in</strong>clude practice standards, protocols, practice parameters, algorithms,<br />

care plans, and critical pathways. Each term represents a m<strong>in</strong>or variation which<br />

fulfills the elements of the IOM def<strong>in</strong>ition.<br />

Guidel<strong>in</strong>es may also serve as criteria for medical review, performance measures<br />

and reimbursement, although these are generally applied after care has been<br />

provided to assess its quality. In these cases, their use does not fit the criterion of<br />

assisted decision-mak<strong>in</strong>g that would allow them to meet the def<strong>in</strong>ition of practice<br />

guidel<strong>in</strong>es.<br />

11.3 The Guidel<strong>in</strong>e Lifecycle<br />

The guidel<strong>in</strong>e lifecycle is a series of stages from conception to revision. 4<br />

The lifecycle beg<strong>in</strong>s with the perception of a need to standardize processes.<br />

This perception may be based on recognition of variations <strong>in</strong> care delivery, of<br />

the need to clarify the evidence base support<strong>in</strong>g a topic or an expectation of cost<br />

sav<strong>in</strong>gs.<br />

Once the need for a guidel<strong>in</strong>e is recognized and resources are assembled,<br />

author<strong>in</strong>g beg<strong>in</strong>s. Author<strong>in</strong>g is translation of published evidence and expert<br />

consensus <strong>in</strong>to formal policy statements. Ideally, a multidiscipl<strong>in</strong>ary group of<br />

stakeholders and experts def<strong>in</strong>e a set of questions that the guidel<strong>in</strong>e addresses.<br />

Follow<strong>in</strong>g an exhaustive literature search and a filter<strong>in</strong>g process to select wellperformed<br />

and relevant studies that address the cl<strong>in</strong>ical questions, guidel<strong>in</strong>e<br />

authors develop a draft document that def<strong>in</strong>es appropriate diagnosis and management,<br />

based on synthesis of evidence and weigh<strong>in</strong>g of anticipated benefits,<br />

risks, harms, and costs. Several drafts are prepared and critiqued until consensus<br />

is reached. The penultimate draft is then circulated to nonparticipant stakeholders<br />

for critical review and revision if necessary and f<strong>in</strong>ally sanctioned by the<br />

sponsor<strong>in</strong>g organization.<br />

In dissem<strong>in</strong>ation, the guidel<strong>in</strong>e is circulated to its <strong>in</strong>tended audience <strong>in</strong> a nontargeted<br />

manner. Guidel<strong>in</strong>es are published <strong>in</strong> professional journals, sent directly<br />

to stakeholders who may be <strong>in</strong>terested <strong>in</strong> the topic and/or made available onl<strong>in</strong>e<br />

via sponsor<strong>in</strong>g organizations. The Agency for <strong>Health</strong>care Research and Quality<br />

ma<strong>in</strong>ta<strong>in</strong>s the National Guidel<strong>in</strong>es Clear<strong>in</strong>ghouse on a website (www.guidel<strong>in</strong>es.<br />

gov) that archives and <strong>in</strong>dexes almost 2,000 practice guidel<strong>in</strong>es.<br />

Guidel<strong>in</strong>e implementation is the assimilation of recommendations <strong>in</strong>to cl<strong>in</strong>ical<br />

care processes, through operationalization <strong>in</strong> specific cl<strong>in</strong>ical sett<strong>in</strong>gs to<br />

improve local outcomes of care. Guidel<strong>in</strong>e implementation <strong>in</strong>to <strong>in</strong>formation<br />

systems requires encod<strong>in</strong>g to make them computer-executable.<br />

Guidel<strong>in</strong>e ma<strong>in</strong>tenance by representatives of the author<strong>in</strong>g organization focuses<br />

on revision of recommendations based on changes <strong>in</strong> the evidence base. Most<br />

organizations specify a time period after which each guidel<strong>in</strong>e must be reviewed,<br />

revised if necessary or retired.
