cardiovascular problems: patent ductus arteriosus (PDA) and hypotension. PDA is<br />

a persistence of an essential fetal connection (the ductus arteriosus) between the<br />

pulmonary artery and aorta that normally closes with<strong>in</strong> 12–24 h <strong>in</strong> term neonates.<br />

In premature <strong>in</strong>fants, the PDA can worsen respiratory distress and lower systemic<br />

blood pressure. Various medical and surgical therapies are used to close the duct. 29<br />

Hypotension, another complication of prematurity, is a poorly def<strong>in</strong>ed entity <strong>in</strong><br />

extremely preterm <strong>in</strong>fants. At this time, neonatalogists commonly attempt to keep<br />

the mean blood pressure at least the gestational age dur<strong>in</strong>g the first several days<br />

after birth. 30<br />

Mean blood pressure = DBP + 1/3 * (SBP − DBP) 31<br />

The medical management of hypotension <strong>in</strong>cludes:<br />

Cont<strong>in</strong>uous <strong>in</strong>travenous <strong>in</strong>fusions of vasopressors (dopam<strong>in</strong>e, dobutam<strong>in</strong>e, ep<strong>in</strong>ephr<strong>in</strong>e)<br />

are adm<strong>in</strong>istered on a microgram per kilogram per m<strong>in</strong>ute rate (unlike<br />

narcotics, which are <strong>in</strong>fused on a microgram per kilogram per hour rate). Manual<br />

calculation of <strong>in</strong>fusion rates and doses is error-prone. Errors can be reduced through<br />

the design and mandated use of calculators 32 that can be used <strong>in</strong> stressful situations.<br />

In addition to calculators, the mandated use of standard concentrations for<br />

cont<strong>in</strong>uous <strong>in</strong>fusion medications 33 by the Jo<strong>in</strong>t Commission, which limits available<br />

concentrations of medications, thus reduc<strong>in</strong>g pharmacy preparation errors. The large<br />

variations <strong>in</strong> neonatal weights may create fluid overload for premature <strong>in</strong>fants, but<br />

this may be offset by the use of computerized “smart” pumps (See Chapter 28).<br />

Steroids (<strong>in</strong> particular hydrocortisone), dosed by weight (mg/kg/day) or by body<br />

surface area (BSA) may help stabilize blood pressure (us<strong>in</strong>g calculator support):<br />

Neonatal Estimated BSA (m 2 ) = 0.05 × Wt (Kg) + 0.05<br />

4.5.2.3 Neurologic Immaturity and Vulnerability<br />

Extremely premature <strong>in</strong>fants are at risk for bra<strong>in</strong> damage due to hypoxic and<br />

ischemic <strong>in</strong>sults to the develop<strong>in</strong>g bra<strong>in</strong> and nervous system. Infants less than<br />

34 weeks gestational age (at birth) are at significant risk for <strong>in</strong>traventricular<br />

hemorrhage (IVH). 23 Hemorrhages, detected us<strong>in</strong>g cranial ultrasonography, can<br />

range from very small to catastrophic, lethal hemorrhagic <strong>in</strong>farctions, 34 which<br />

may create posthemorrhagic hydrocephalus, requir<strong>in</strong>g neurosurgical <strong>in</strong>tervention.<br />

Periventricular leukomalacia (PVL), result<strong>in</strong>g from blood flow <strong>in</strong>stability and<br />

exposure to <strong>in</strong>fection among extremely premature <strong>in</strong>fants, may be detected by cranial<br />

ultrasound and places an <strong>in</strong>fant at significant risk for long term developmental<br />

problems. The presence of <strong>in</strong>traventricular hemorrhages, periventricular leukomalacia,<br />

or hydrocephalus is vitally important for future medical and developmental<br />

care and this <strong>in</strong>formation must be clearly conveyed to future physicians and allied<br />

developmental professionals.
