26.1 Introduction<br />

Institutional adoption of computerized provider order entry (CPOE) and cl<strong>in</strong>ical<br />

decision support (CDS) has been advocated by the Institute of Medic<strong>in</strong>e (IOM)<br />

and others to reduce and prevent medication prescrib<strong>in</strong>g errors as part of a global<br />

approach to safety (Table 26.1) 1 that <strong>in</strong>cludes: foster<strong>in</strong>g an open, nonpunitive, and<br />

goal-directed culture and environment for learn<strong>in</strong>g and teach<strong>in</strong>g safety improvement,<br />

understand<strong>in</strong>g, and anticipat<strong>in</strong>g human behaviors and limitations, tra<strong>in</strong><strong>in</strong>g<br />

and work<strong>in</strong>g as a team, and creat<strong>in</strong>g robust systems for anticipat<strong>in</strong>g and handl<strong>in</strong>g<br />

the unexpected.<br />

26.2 Def<strong>in</strong>itions<br />

<strong>Computer</strong>ized Provider Order Entry (CPOE, also called POE, <strong>Computer</strong>ized<br />

Physician Order Entry or Care Provider Order Entry) is the component of a cl<strong>in</strong>ical<br />

<strong>in</strong>formation system that allows prescribers to enter cl<strong>in</strong>ical orders directly <strong>in</strong>to a<br />

computer for electronic process<strong>in</strong>g and transmission to appropriate departments<br />

and/or <strong>in</strong>dividuals for completion. Ambulatory CPOE (ACPOE) may connect<br />
