30.1 Introduction<br />

The ability to detect errors <strong>in</strong> medic<strong>in</strong>e is an important start<strong>in</strong>g po<strong>in</strong>t for programmatic<br />

<strong>in</strong>terventions aimed at improv<strong>in</strong>g patient safety. While <strong>in</strong>formation technology<br />

has the ability to improve many aspects of healthcare, the optimization of error<br />

report<strong>in</strong>g can ultimately improve error reduction because of the focus it br<strong>in</strong>gs to<br />

system defects.<br />

30.2 Quality, Safety, and Errors<br />

The healthcare <strong>in</strong>dustry has become <strong>in</strong>creas<strong>in</strong>gly focused on outcomes. The<br />

Institute of Medic<strong>in</strong>e (IOM) reports on healthcare quality were <strong>in</strong>tended to improve<br />

the healthcare services delivered to <strong>in</strong>dividuals and populations, 1–3 and discussions<br />

on quality, safety and errors are more prom<strong>in</strong>ent as a result.<br />

30.2.1 Quality<br />

Quality of care is def<strong>in</strong>ed <strong>in</strong> terms of health outcomes for <strong>in</strong>dividuals and populations.<br />

Thus, <strong>in</strong>stitutional improvements <strong>in</strong> care delivery should result <strong>in</strong> measurable<br />

improvements <strong>in</strong> outcomes. For adult medic<strong>in</strong>e, core quality measures are well<br />
