and Quality (AHRQ) Web site has many grants for <strong>in</strong>dividuals <strong>in</strong>terested <strong>in</strong> apply<strong>in</strong>g<br />

technology to their practice. Most <strong>in</strong>volve studies us<strong>in</strong>g technology for certa<strong>in</strong><br />

cl<strong>in</strong>ical conditions. Others <strong>in</strong>volve large organizations evaluat<strong>in</strong>g the effectiveness<br />

of technology to solve particular problems. 13<br />

16.4.4.5 Network Membership Benefit<br />

Practices associated with multiple hospital systems often see value <strong>in</strong> us<strong>in</strong>g practice<br />

management and electronic health record systems that are compatible or <strong>in</strong>teroperable<br />

with the systems used by those <strong>in</strong>stitutions. Mutual benefits between hospitals<br />

and office-based practices <strong>in</strong>clude shar<strong>in</strong>g of demographic, bill<strong>in</strong>g and payment<br />

data and easier track<strong>in</strong>g of referrals and more accurate bill<strong>in</strong>g. After the <strong>in</strong>itial costs<br />

of development by hospitals, technology becomes a market<strong>in</strong>g tool that can be used<br />

to build loyalty at low cost to private practices.<br />

An issue that should be considered is the effect of Stark (or “anti-kickback”)<br />

legislation 14 on adoption. Stark legislation had h<strong>in</strong>dered growth of technology <strong>in</strong><br />

practices that do not have the f<strong>in</strong>ancial resources to <strong>in</strong>vest. This is of significance <strong>in</strong><br />

IT obta<strong>in</strong>ed as a network membership benefit. Aga<strong>in</strong>, legal counsel may be helpful<br />

<strong>in</strong> navigat<strong>in</strong>g these issues.<br />

16.4.5 Expenses – Ongo<strong>in</strong>g and Operational<br />

16.4.5.1 Software Licenses<br />

Software licenses are certificates, sometimes l<strong>in</strong>ked to access and services that<br />

allow practices to use purchased software (such as practice management and electronic<br />

health record systems, cl<strong>in</strong>ical decision support, periodicals, cl<strong>in</strong>ical journals,<br />

or diagnostic services). Group licenses (such as through professional associations<br />

or community practice consortia (IPAs) for a shared or commonly used application<br />

may reduce <strong>in</strong>dividual costs at a bulk rate.<br />

16.4.5.2 Tra<strong>in</strong><strong>in</strong>g and Technical Support<br />

Tra<strong>in</strong><strong>in</strong>g staff to use applications properly can be an additional expense if it is not<br />

<strong>in</strong>cluded as part of the purchase price. Tra<strong>in</strong><strong>in</strong>g costs may exceed the software costs<br />

when travel, hous<strong>in</strong>g, and consultation are <strong>in</strong>cluded, but may be mitigated if onl<strong>in</strong>e<br />

tra<strong>in</strong><strong>in</strong>g is available.<br />

Technical support should be considered <strong>in</strong> terms of how and when service is<br />

scheduled (Telephone? Onl<strong>in</strong>e? 24/7/365? Workdays only? Immediate response?<br />

Can ma<strong>in</strong>tenance or repairs be handled onl<strong>in</strong>e (onsite repair costs may be higher)?).
