20 Telemedic<strong>in</strong>e <strong>Applications</strong> <strong>in</strong> <strong>Pediatric</strong>s 283<br />

For telemedic<strong>in</strong>e systems that are <strong>in</strong> regular use, on-site technical support is<br />

required and must be able to work closely with cl<strong>in</strong>icians, equipment vendors, telephone<br />

companies, and connection providers. Ideal services customize telemedic<strong>in</strong>e<br />

solutions that successfully comb<strong>in</strong>e real-time videoconferenc<strong>in</strong>g, digital report<strong>in</strong>g,<br />

image storage and management, data base <strong>in</strong>tegration, prompt technical support,<br />

and easy implementation of software updates.<br />

Physician champions and adm<strong>in</strong>istrative leaders are necessary to promote use of<br />

this technology to physicians who may be resistant to change. Advocacy for any<br />

telemedic<strong>in</strong>e <strong>in</strong>itiative must be active on both sides of the connection. Leadership<br />

roles <strong>in</strong> overcom<strong>in</strong>g barriers to acceptance <strong>in</strong>clude removal of technical, organizational<br />

and legal blocks and adm<strong>in</strong>istrative simplification of legal issues surround<strong>in</strong>g<br />

telemedic<strong>in</strong>e.<br />

Medical education support is helpful <strong>in</strong> <strong>in</strong>creas<strong>in</strong>g adoption. The <strong>in</strong>clusion of<br />

facilitated cont<strong>in</strong>u<strong>in</strong>g medical education credits will improve the attractiveness of<br />

telemedic<strong>in</strong>e programs that <strong>in</strong>clude distance education. Additional added-value<br />

tools will allow users to acquire and to create content, to <strong>in</strong>corporate multimedia<br />

and to access evidence easily.<br />

A challenge for a telemedic<strong>in</strong>e practice is to generate sufficient revenue to<br />

be self-susta<strong>in</strong><strong>in</strong>g. However, significant fund<strong>in</strong>g opportunities are available<br />

( pr<strong>in</strong>cipally through federal sources), especially for programs that provide support<br />

for underserved rural and <strong>in</strong>ternational areas. A susta<strong>in</strong>able plan for telemedic<strong>in</strong>e<br />

must <strong>in</strong>clude fund<strong>in</strong>g for ma<strong>in</strong>tenance, user support and upgrad<strong>in</strong>g (hardware, software,<br />

connectivity, and education content).<br />

20.4 <strong>Pediatric</strong> Telemedic<strong>in</strong>e <strong>Applications</strong><br />

Cl<strong>in</strong>ical pediatric telemedic<strong>in</strong>e applications can be found <strong>in</strong> almost every<br />

subspecialty, 8 <strong>in</strong>clud<strong>in</strong>g cardiology, 9 radiology, neurology, 10,11 neonatal care, genetics,<br />

emergency medic<strong>in</strong>e, pathology, 12 mental health, 13 hematology, oncology, 14<br />

dermatology, otolaryngology, ophthalmology, orthopedic surgery, urology, and<br />

general surgery. Telemedic<strong>in</strong>e applications have also been developed for distance<br />

learn<strong>in</strong>g, home care, correctional care, <strong>in</strong>ternational collaboration, humanitarian<br />

assistance, rural health, 15 military health, Native American health, pharmacy, and<br />

consumer health.<br />

20.4.1 Cardiology<br />

The earliest cl<strong>in</strong>ical publications on tele-cardiology were reported <strong>in</strong> pediatric<br />

populations <strong>in</strong> 1989 and 1993. 16 Live transmission of neonatal echocardiograms is<br />

accurate and impacts positively on patient management <strong>in</strong>clud<strong>in</strong>g: facilitat<strong>in</strong>g timely<br />

transport of critically ill children with heart disease, prevent<strong>in</strong>g unnecessary transport<br />

(patients diagnosed not to have heart disease), improv<strong>in</strong>g cost-effectiveness
