levels to prevent toxicity. It is also common to adm<strong>in</strong>ister fol<strong>in</strong>ic acid (Leucovor<strong>in</strong>)<br />

which is an antidote to methotrexate and rescues healthy cells from methotrexate’s<br />

toxic effects at the conclusion of therapy. We can exam<strong>in</strong>e how CPOE could be<br />

used to develop an error-prevention strategy to prevent methotrexate toxicity.<br />

CPOE applications could elim<strong>in</strong>ate the task of cumbersome calculations for<br />

body surface area by recall<strong>in</strong>g height and weight data stored as a component of<br />

other tasks. The application could also calculate appropriate methotrexate doses<br />

thereby elim<strong>in</strong>at<strong>in</strong>g the task of that manual calculation. CPOE could also replace<br />

the action of handwrit<strong>in</strong>g with the action of typ<strong>in</strong>g or selection from pick-lists.<br />

This would be expected to have the result of improv<strong>in</strong>g legibility and prevent<strong>in</strong>g<br />

certa<strong>in</strong> handwrit<strong>in</strong>g <strong>in</strong>terpretation errors. A multitude of functions could also be<br />

facilitated electronically. For example, memory could be facilitate through the<br />

use of order sets that comb<strong>in</strong>e orders for leucovor<strong>in</strong> and methotrexate level monitor<strong>in</strong>g<br />

along with the methotrexate order itself. Perception of potentially worrisome<br />

laboratory values that might <strong>in</strong>fluence the medication order<strong>in</strong>g could be<br />

facilitated through the use of alerts. Judgment could be facilitated by mak<strong>in</strong>g drug<br />

<strong>in</strong>formation, patient-specific <strong>in</strong>formation such as history or laboratory values, or<br />

disease management protocols available at the po<strong>in</strong>t of order<strong>in</strong>g. The detection<br />

of abnormalities such as a high methotrexate level could be improved by forc<strong>in</strong>g<br />

or automat<strong>in</strong>g the order<strong>in</strong>g of drug level monitor<strong>in</strong>g at the time of medication<br />

order<strong>in</strong>g. Rescue could be better assured if <strong>in</strong>formation systems also monitored<br />

the results of drug level monitor<strong>in</strong>g and provided alerts to appropriate caregivers<br />

that the patient is <strong>in</strong> danger and requires immediate changes <strong>in</strong> therapy. Failures<br />

to rescue could be reduced further were that <strong>in</strong>formation system to require a<br />

timely response to the alert and escalate the notification to the next caregiver <strong>in</strong><br />

l<strong>in</strong>e should no response be received.<br />

Evidence supports the potential benefits of <strong>in</strong>formatics solutions <strong>in</strong> reduc<strong>in</strong>g<br />

error. For example, computerized physician order entry systems can reduce medication<br />

prescrib<strong>in</strong>g errors. 23–26 In addition, computerized decision support systems<br />

can improve performance and diagnosis. 27–29 It is important to recognize, however,<br />

that whereas it is possible to conceive, design, and build <strong>in</strong>formatics solutions<br />

<strong>in</strong>tended to reduce error and improve performance, implementation does not<br />

guarantee impact. Systems can fail to have their <strong>in</strong>tended impact for any number<br />

of reasons <strong>in</strong>clud<strong>in</strong>g users ignor<strong>in</strong>g or reject<strong>in</strong>g computerized advice, 30 hav<strong>in</strong>g difficulty<br />

us<strong>in</strong>g the systems, 31 or challenges <strong>in</strong> <strong>in</strong>tegrat<strong>in</strong>g the tools <strong>in</strong>to caregiver’s<br />

workflow. 32 One strategy for optimiz<strong>in</strong>g the impact of <strong>in</strong>formatics solutions is to<br />

design and implement consider<strong>in</strong>g the failure modes of the <strong>in</strong>formation systems<br />

themselves.<br />

29.6.2 Failure Modes <strong>in</strong> <strong>Informatics</strong> Solutions<br />

Although <strong>in</strong>formation systems may confer substantial benefits <strong>in</strong> reduc<strong>in</strong>g error,<br />

they often substitute different tasks and functions than would be required <strong>in</strong> com-
