refills). Services to streaml<strong>in</strong>e and improve care <strong>in</strong>clude: onl<strong>in</strong>e pre-visit forms (that<br />

can be completed through <strong>in</strong>-office kiosks, sav<strong>in</strong>g on paper, clipboards and time spent<br />

on redundant paperwork), Web-based patient surveys, onl<strong>in</strong>e health monitor<strong>in</strong>g, and<br />

patient teach<strong>in</strong>g that have been shown to be effective. It is believed that physicians<br />

can use these tools to <strong>in</strong>crease productivity by reduc<strong>in</strong>g redundant data entry.<br />

Web portals empower patients by allow<strong>in</strong>g them to manage their appo<strong>in</strong>tments,<br />

refills, and referrals. Through portals, patients may: access laboratory and imag<strong>in</strong>g<br />

test results as they become available, update their medication lists, drug and food<br />

allergies and medical histories and communicate directly with their providers via<br />

secure messag<strong>in</strong>g. Patients may also manage and their patient accounts with practices.<br />

Many EMR vendors currently provide Web portal services to participat<strong>in</strong>g<br />

practices, with practitioners show <strong>in</strong>creas<strong>in</strong>g <strong>in</strong>terest.<br />

Example:<br />

The follow<strong>in</strong>g Web portal example, provided by an electronic medical record vendor,<br />

shows a (test) patient’s visit dates, immunization history, allergy history and allows<br />

the patient/family to schedule an appo<strong>in</strong>tment for health ma<strong>in</strong>tenance and to request<br />

<strong>in</strong>formation:<br />

In addition to patient appo<strong>in</strong>tment schedul<strong>in</strong>g/cancellation, some portals offer<br />

patients onl<strong>in</strong>e request forms for prescription refills, enrollment/registration and<br />

documents, onl<strong>in</strong>e review and updat<strong>in</strong>g of health and demographic <strong>in</strong>formation,<br />

e-mail rem<strong>in</strong>ders, secure messag<strong>in</strong>g to providers, onl<strong>in</strong>e news from the practice,<br />

onl<strong>in</strong>e consultation and onl<strong>in</strong>e surveys.<br />

Others offer virtual office visit, symptom assessment, onl<strong>in</strong>e bill payment, “Ask<br />

a Doctor” function and laboratory result report retrieval.<br />

These examples demonstrate currently available functionality. Future functions<br />

that Web portals may provide patients <strong>in</strong>clude: Medical Home functions to help<br />

families to self-manage and co-manage health ma<strong>in</strong>tenance (immunizations and<br />

growth) and chronic disease (blood glucose measures <strong>in</strong> diabetes and peak flow<br />

measurements <strong>in</strong> asthma) <strong>in</strong>formation. There is evidence that this type of <strong>in</strong>formation<br />

access can improve patient safety and thereby reduce health care costs (through<br />

decreased hospital and emergency department visits) (Fig. 24.3). 25–29<br />
