25.1 Introduction<br />

The term “medication process” <strong>in</strong>cludes all aspects of the management of<br />

pharmaceuticals used <strong>in</strong> the care of patients: drug manufacture (creat<strong>in</strong>g, packag<strong>in</strong>g,<br />

and assur<strong>in</strong>g the quality and safety of medications for human use), pharmaceutical<br />

procurement (acquir<strong>in</strong>g, stor<strong>in</strong>g, and manag<strong>in</strong>g medications for patient care), medication<br />

reconciliation (identify<strong>in</strong>g, verify<strong>in</strong>g, and realign<strong>in</strong>g a patient’s current drug<br />

regimen with the <strong>in</strong>tended care plan when a change <strong>in</strong> medical care (admission,<br />

discharge, transfer) occurs) and medication use. Medication use is the process of<br />

prescrib<strong>in</strong>g, order<strong>in</strong>g, compound<strong>in</strong>g, dispens<strong>in</strong>g, and adm<strong>in</strong>ister<strong>in</strong>g drugs, monitor<strong>in</strong>g<br />

(and document<strong>in</strong>g) their effects on patients. It is the last aspect of which this<br />

section is an overview, and which is the focus of subsequent sections.<br />

The complexity of each of the above-mentioned processes, with<strong>in</strong> their respective<br />

complex environments (chemical manufactur<strong>in</strong>g plants, pharmacies, hospitals, and<br />

cl<strong>in</strong>ics) makes medication processes <strong>in</strong>herently error-prone. Of these processes, the<br />

largest negative impact on patient health comes from errors <strong>in</strong> mediation use. Illegible<br />

handwrit<strong>in</strong>g, numerous human hand-offs of <strong>in</strong>structions, complex patient data, critical<br />

drug dose <strong>in</strong>formation and ever-chang<strong>in</strong>g formularies of drugs with vary<strong>in</strong>g degrees<br />

of potency and toxicity all contribute to the vulnerabilities and errors <strong>in</strong> medication<br />

use, and to the challenges of efficiently and safely manag<strong>in</strong>g them. Not surpris<strong>in</strong>g,<br />

adverse drug events (ADEs) result <strong>in</strong> more than 770,000 <strong>in</strong>juries and deaths each<br />

year <strong>in</strong> the US and cost up to $5.6 million per hospital. 1<br />
