must participate <strong>in</strong> plann<strong>in</strong>g of health care IT <strong>in</strong> <strong>in</strong>tegrated systems by:<br />

Educat<strong>in</strong>g IT professionals of the unique and special needs of CSHCN<br />

Participat<strong>in</strong>g <strong>in</strong> the design and adoption of IT tools that facilitate data collection<br />

from patients, improve access to care and allow timely analysis of patient<br />

outcomes<br />

Demonstrat<strong>in</strong>g to policy makers and other stakeholders by us<strong>in</strong>g IT to collect<br />

data on unmet needs that low reimbursements and related issues have led to difficulties<br />

<strong>in</strong> provid<strong>in</strong>g adequate care for CSHCN<br />

Serv<strong>in</strong>g as local and national champions to garner grassroots support for the<br />

needs of CSHCN to be represented <strong>in</strong> IT development that supports the Medical<br />

Home and all its functions <strong>in</strong> provid<strong>in</strong>g chronic care management<br />

The grow<strong>in</strong>g population of CSHCN will require resources, expertise and development<br />

of <strong>in</strong>formation tools to improve the health and to protect the rights of this<br />

vulnerable group of children.<br />
