26.3 CPOE/CDS Functions<br />

In <strong>in</strong>patient sett<strong>in</strong>gs, CPOE and associated CDS provide prescrib<strong>in</strong>g support to a<br />

cl<strong>in</strong>ician (physician, nurse practitioner, nurse, physician assistant) who is authorized<br />

to order medications, laboratory and radiology tests, and treatments, for patients:<br />

The authorized cl<strong>in</strong>ician establishes a therapeutic relationship with a patient and<br />

collects data (history, physical exam<strong>in</strong>ation, laboratory test results) to determ<strong>in</strong>e<br />

a diagnosis or problem list for the patient.<br />

Based on the diagnosis or problem list <strong>in</strong>ferred from the collected data, the<br />

cl<strong>in</strong>ician determ<strong>in</strong>es the need for further test<strong>in</strong>g or specific therapies for<br />

the patient, accord<strong>in</strong>g to medical evidence, practice guidel<strong>in</strong>es and cl<strong>in</strong>ical constra<strong>in</strong>ts<br />

(formulary availability, drug <strong>in</strong>teractions).<br />

The cl<strong>in</strong>ician formulates, orders, and documents tests and therapies (us<strong>in</strong>g CPOE)<br />

that are transmitted to ancillary departments or the pharmacist and nurse for completion.<br />

CDS guides the cl<strong>in</strong>ician by guid<strong>in</strong>g choices and data entry, by provid<strong>in</strong>g<br />

knowledge-based prompts and access to <strong>in</strong>formation about drugs and the patient.<br />

In response to test results, alerts, and requests for correction and clarification,<br />

the cl<strong>in</strong>ician adjusts orders and their documentation as required.<br />

CPOE (depend<strong>in</strong>g on the degree of sophistication and CDS) supports and improves<br />

the reliability of the prescrib<strong>in</strong>g/order<strong>in</strong>g step by:<br />

Reduc<strong>in</strong>g illegibility and transcription errors by elim<strong>in</strong>at<strong>in</strong>g handwrit<strong>in</strong>g<br />

Standardiz<strong>in</strong>g identifiers, names, and codes of cl<strong>in</strong>ical entities: drugs, tests,<br />

and patients by us<strong>in</strong>g data dictionaries and<br />

L<strong>in</strong>k<strong>in</strong>g patient-specific data and <strong>in</strong>formation to the order<strong>in</strong>g process<br />

Provid<strong>in</strong>g evidence-based order sets<br />

Automat<strong>in</strong>g calculations<br />

Provid<strong>in</strong>g alerts and rem<strong>in</strong>ders<br />

Screen<strong>in</strong>g for populations at risk 5<br />

26.4 <strong>Pediatric</strong>-Specific Technical Features of CPOE/CDS<br />

Different populations of children have different vulnerabilities and safety needs<br />

(Chapters 3–8). With<strong>in</strong> pediatric care environments, features that should be taken<br />

<strong>in</strong>to consideration when design<strong>in</strong>g or choos<strong>in</strong>g CPOE/CDS systems <strong>in</strong>clude:<br />

Universal weight-based dos<strong>in</strong>g<br />

� Automatic drug dose calculations and round<strong>in</strong>g<br />

� Drug dictionaries with up-to-date pediatric-specific dose <strong>in</strong>formation<br />

� Dos<strong>in</strong>g alerts, rem<strong>in</strong>ders, and warn<strong>in</strong>gs that are age, weight, and drug specific<br />

� Override (“break the glass”) options for specific cases and prescribers<br />

(very high or low weight children, impaired drug absorption or clearance,<br />

differential tolerances <strong>in</strong> pa<strong>in</strong> management)
