discussion with<strong>in</strong> the pediatric and <strong>in</strong>formatics literature. 12 Subsequent studies 13,14<br />

support that CPOE does not <strong>in</strong>crease mortality <strong>in</strong> pediatric critical care.<br />

26.8.3 Study 3: CDS <strong>in</strong> <strong>Pediatric</strong> Oncology<br />

<strong>Pediatric</strong> oncology presents challenges that <strong>in</strong>crease the likelihood of medication<br />

errors 15 that result <strong>in</strong> patient harm (<strong>in</strong> an already vulnerable population). Issues that<br />

contribute to the complexity of the problems <strong>in</strong> car<strong>in</strong>g for this particularly vulnerable<br />

class of patients <strong>in</strong>clude:<br />

Drug dosage calculation challenges (dependent on age, weight, and body surface<br />

area)<br />

Inclusion of many patients <strong>in</strong> cl<strong>in</strong>ical trials (<strong>in</strong>clud<strong>in</strong>g issues of schedul<strong>in</strong>g,<br />

track<strong>in</strong>g, and bill<strong>in</strong>g)<br />

Complex protocols that comb<strong>in</strong>e time-critical surgery, chemotherapy (that may<br />

be delivered to different body cavities: <strong>in</strong>tra-thecal, etc.) and radiation treatments<br />

of vary<strong>in</strong>g <strong>in</strong>tensities 16<br />

The need for cont<strong>in</strong>uity of care that transcends <strong>in</strong>patient/outpatient patient care<br />

environment dist<strong>in</strong>ctions 17<br />

Interruptions, adjustments and delays to <strong>in</strong>dividual treatment schedules due to<br />

<strong>in</strong>tercurrent illnesses, some as complications of therapy<br />

The narrow therapeutic profile and high potential for acute and cumulative<br />

toxicities of the agents used<br />

Risks of preventable long-term effects of therapy that may create needs for<br />

modifications of treatment protocols and <strong>in</strong>dividual schedules<br />

Because of these complexities, global approaches have been used to exam<strong>in</strong>e the risks<br />

at different steps of order<strong>in</strong>g chemotherapy 18 and oncology laboratory test<strong>in</strong>g 19 and<br />

of the global prescrib<strong>in</strong>g-pharmacy process<strong>in</strong>g-adm<strong>in</strong>istration process. 20 The design,<br />

application, and evaluation of cl<strong>in</strong>ical decision and workflow support for all phases<br />

of pediatric oncology rema<strong>in</strong>s an important area of <strong>in</strong>quiry and development.<br />
