SECTION I:<br />

GATHERING INFORMATION/PROBLEM IDENTIFICATION<br />

PURPOSE: To gather in<strong>for</strong>mation from all team members.<br />

DIRECTONS: All team members should receive a copy of Section I-A <strong>for</strong> review prior to<br />

the planning meeting. A group memory of thoughts and ideas will be<br />

created at the planning meeting on large paper or laminated recording<br />

sheets <strong>for</strong> all to see.<br />

TIME: 10 minutes to share ideas and thoughts.<br />

Section I, Part A: Parent/Teacher Concerns:<br />

♦ What are your child’s/student’s interests?<br />

♦ What motivates your child/student?<br />

♦ Are there activities at school in which you feel your child/student needs to increase<br />

his/her participation or independence?<br />

♦ Is your child/student experiencing any problems at school or expressing any<br />

frustrations?<br />

♦ How could assistive technology enhance your child’s/student’s learning,<br />

participation and/or independence?<br />

♦ Does your child/student use any assistive devices at school?<br />

♦ Describe your child’s/student’s feeling about using these devices at school.<br />

♦ Do you feel the devices are meeting his/her needs?<br />

♦ Does your child/student use any assistive devices at home?<br />

♦ Do you feel the devices are meeting his/her needs?<br />

♦ Other thoughts/ideas:<br />
