FOLLOW-UP PLAN<br />

PURPOSE: To determine current status and effectiveness of implementation<br />

plan at a later date.<br />

DIRECTIONS: The team should discuss the issues/successes of the plan.<br />

TIME: 20 minutes.<br />

Section V, Part A: Evaluation Of Implementation Plan<br />

♦ Review the implementation plan, has everything been implemented?<br />

♦ Is the assistive technology utilized as stated in the implementation plan?<br />

♦ Is the assistive technology being used to address the student’s IEP goals and objectives?<br />

♦ How has the student reacted to use of the assistive technology?<br />

♦ How has the assistive technology functioned?<br />

♦ Is staff com<strong>for</strong>table or need training with the use of assistive technology?<br />

♦ Does it appear, based on the student’s functional use of assistive technology, that any<br />

additional adaptations to the assistive technology are needed?<br />

♦ Are there any additional problems which may need to be addressed?<br />

♦ Does the student need extended assessment in area of assistive technology?<br />

♦ When will use of the assistive technology be reviewed again to identify<br />

effectiveness/functional limitations/problems?<br />
