Required Admission Materials<br />

Listed below are the required admission materials for the<br />

Associate of Science in Dental Hygiene and the Bachelor<br />

of Science in Dental Hygiene, entry level.<br />

Dental Hygiene Program Application<br />

This is different than the <strong>Indiana</strong> <strong>University</strong> application,<br />

is obtained from the Office of Dental Education, and must<br />

be returned to this office. This application is valid only<br />

for the IU <strong>South</strong> <strong>Bend</strong> Dental Hygiene degree program,<br />

and not other <strong>Indiana</strong> <strong>University</strong> Dental Hygiene degree<br />

programs.<br />

Curriculum Information Sheet<br />

This is obtained from the Office of Dental Education and<br />

returned to that office. Curriculum information sheets or<br />

similar forms from other programs are not accepted.<br />

College Transcript<br />

An official transcript from all colleges and universities<br />

attended must be submitted to the Dental Hygiene degree<br />

program in addition to the transcripts required by the IU<br />

<strong>South</strong> <strong>Bend</strong> Office of Admissions. An official transcript<br />

for course work completed at <strong>Indiana</strong> <strong>University</strong> is not<br />

required.<br />

Documentation of Dental Hygiene Observations<br />

Applicants must observe at least two different dental<br />

hygienists in at least two different practice settings for<br />

a total of eight hours. To document the observation,<br />

request the dental hygienist to provide the following<br />

information on their office letterhead: the applicants<br />

name, date, time of observation, and their signature.<br />

An appointment for dental hygiene treatment at the IU<br />

<strong>South</strong> <strong>Bend</strong> Dental Clinic during the academic year may<br />

be substituted for four hours of observation.<br />

Note: Students must complete all required prerequisite<br />

course work before they are eligible to enroll in Dental<br />

Hygiene classes.<br />

Applicants selected for regular admission and who<br />

meet the February 1 application deadline are notified<br />

of their admission status. To hold a place in the class,<br />

applicants must return the confirmation form and<br />

register for classes by July 15. Applications received after<br />

the February 1 deadline are accepted. Once acceptance is<br />

finalized, students enrolled at other <strong>Indiana</strong> <strong>University</strong><br />

campuses must complete an intercampus transfer before<br />

they can enroll and register for classes at IU <strong>South</strong> <strong>Bend</strong>.<br />

Admission Policies Bachelor of Science in<br />

Dental Hygiene, Entry Level<br />

An IU <strong>South</strong> <strong>Bend</strong> admissions application should be<br />

submitted to the Office of Admissions. Upon formally<br />

being admitted to IU <strong>South</strong> <strong>Bend</strong>, students must meet<br />

with an advisor from the College of Health Sciences to<br />

set up an academic plan of study. Upon completion of<br />

the prerequisite courses, application for admission to the<br />

Dental Hygiene degree program should be completed<br />

and returned to the dental education office along with<br />

the supporting documents by February 1. If a student<br />

is enrolled for the spring semester, a transcript of work<br />

completed must be sent to the dental education office at the<br />

end of the semester. Acceptance cannot be granted until<br />

this is received. All prerequisites must be completed before<br />

entrance into the program.<br />

Admission Policies Bachelor of Science in<br />

Dental Hygiene, Degree Completion<br />

Graduates from an accredited degree program holding<br />

the Associate of Science in Dental Hygiene, and a current<br />

<strong>Indiana</strong> dental hygiene license must submit an application<br />

to be admitted to the Bachelor of Science in Dental Hygiene<br />

degree program after meeting with an academic advisor.<br />

Applicants who receive Dental Hygiene degrees from<br />

accredited programs other than those offered by <strong>Indiana</strong><br />

<strong>University</strong> will be considered transfer students for the<br />

purpose of fulfilling general-education requirements at IU<br />

<strong>South</strong> <strong>Bend</strong>. Courses required for admission may be taken<br />

at any <strong>Indiana</strong> <strong>University</strong> campus or may be accepted as<br />

transfer credit hours from other accredited institutions.<br />

However, priority consideration is given to those students<br />

completing the majority of their course work on the IU <strong>South</strong><br />

<strong>Bend</strong> campus. Admission to the major is highly competitive.<br />

Contact a dental education advisor for more information.<br />

Admissions will be competitive based on the average of<br />

the GPA for professional Dental Hygiene courses and the<br />

GPA for the preprofessional course work. A minimum<br />

GPA of 2.50 in each category is required. Two letters of<br />

recommendation from faculty or dental professionals<br />

are required for non-IU <strong>South</strong> <strong>Bend</strong> degree graduates.<br />

Students with the highest overall GPA and positive letters<br />

of recommendation will be granted admission. A student is<br />

expected to complete the work for a degree within five years<br />

from date of admission.
