0 mario composito di morte, infarto e stroke è risultato signicativamente ridotto da Ticagrelor rispetto a Clopidogrel. Anche tutti gli obiettivi secondari escluso lo stroke e la trombosi su stent sono stati signicativamente meno nel gruppo trattato con Ticagrelor. Questa maggior efcacia è stata osservata qualunque fosse la dose da carico di Clopidogrel impiegata. Tutto ciò non è stato gravato da maggiori sanguinamenti. Nelle linee guida ESC 2011 relative ai pazienti con SCA senza STEMI Ticagrelor viene raccomandato per tutti i pazienti a rischio moderato od alto di eventi ischemici qualunque sia il trattamento precedente e qualunque sia la strategia prevista. Conclusioni Nel corso degli ultimi anni è stata riconosciuta l’importanza dell’inibizione del recettore P2Y12 piastrinico quale target chiave per la terapia antiaggregante delle sindromi coronariche acute. Il Clopidogrel, che si è andato affermando quale antiaggregante di riferimento non è in grado di garantire in tutti i pazienti una inibizione piastrinica efcace ed il suo semplice aumento di dosaggio non appare una strada che garantisca l’obiettivo terapeutico. Pertanto la ricerca farmacologica e clinica hanno individuato nuovi farmaci in grado di migliorare la risposta terapeutica antiaggregante. Prasugrel e Ticagrelor sono entrati a tutti gli effetti tra i farmaci raccomandati seppure per indicazioni diverse. Ticagrelor, nuovo farmaco già attivo n dal suo assorbimento, inibisce in modo reversibile il recettore P2Y12, favorisce un’inibizione piastrinica rapida e potente, e si è dimostrato assai efcace nelle SCA con rischio moderato ed elevato. Ticagrelor , in particolare, ha ridotto la mortalità cardiovascolare e quella per tutte le cause senza causare un aumento di mortalità per sanguinamento. Esistono differenze nel disegno degli studi dei nuovi antiaggreganti che possono rendere difcile il confronto dei dati di mortalità . Con la dovuta attenzione questi nuovi farmaci sono in grado di offrire una potente azione antiaggregante e di migliorare l’outcome rispetto al trattamento con Clopidogrel.
