Full Assessment Form<br />

Having completed the Initial EIA Screening Form (Appendix A) which identified a negative<br />

or potential adverse impact, you are required to complete this Full Assessment form. This<br />

will involve you questioning aspects of a proposed/existing service or policy and<br />

forecasting the likely effect on different groups.<br />

Step 1) What is the impact?<br />

You may want to mention the environment, privacy and dignity, language, transport,<br />

access, signage, local demography…<br />

There is a possibility patients may not be fully involved in the consent process and may not give a<br />

fully informed consent to a procedure due to a failure to provide adequate information in a way<br />

they can access<br />

1a Identify the Equality group(s) that will be affected by the adverse Impact:<br />

1b Think about other socially excluded groups or communities e.g. rural community,<br />

carers, areas of deprivation, low literacy skills.<br />

The main barriers to engaging in the process are not understanding the process (due to youth/old<br />

age, disability, language barriers). This may affect some patients within these groups but belonging<br />

to these groups would not be an issue per se.<br />
