Ventilation Modes<br />

BIPAP<br />

As in all pressure-controlled ventilation modes, the patient is<br />

not prescribed a fixed tidal volume (VT).<br />

The tidal volume results principally from the pressure difference<br />

between the settings for PEEP and Pinsp.<br />

Changes in lung compliance and airways, as well as active<br />

breathing by the patient can lead to changes in tidal volume.<br />

This is a desired effect in this ventilation mode.<br />

With the knowledge that the tidal volume, and therefore the<br />

minute volume, are not constant, the alarm limits for minute volume<br />

must be adjusted with care.<br />

The display of the expiratory measured tidal volume VTe must<br />

be used to set the required difference between the two pressure<br />

levels.<br />

As with SIMV, the time pattern is set using the basic setting<br />

parameters of frequency f and inspiration time Tinsp. The<br />

resultant inspiration and expiration times are calculated by<br />

<strong>EvitaXL</strong> and displayed in the lower half of the screen below the<br />

curve setting. The lower pressure level is set with the PEEP<br />

parameter, while the upper level is set with Pinsp.<br />

When switching over from SIMV to BIPAP mode, and retaining<br />

the time pattern, only the Pinsp setting needs to be changed.<br />

The steepness of the increase from the lower pressure level to<br />

the upper pressure level is controlled by the »Ramp« setting.<br />

The effective time for the increase in pressure cannot become<br />

greater than the set inspiratory time Tinsp.<br />

This precaution ensures that the upper pressure level Pinsp is<br />

reached safely during inspiration. The transition from controlled<br />

ventilation via the weaning phase to fully spontaneous<br />

breathing is achieved by a gradual reduction of inspiratory<br />

pressure Pinsp and/or frequency f.<br />
