<strong>Fiabilidad</strong> <strong>de</strong> <strong>los</strong> <strong>predictores</strong> <strong>clínicos</strong> y <strong>de</strong> <strong>la</strong> <strong>biopsia</strong> <strong>de</strong> <strong>arteria</strong> temporal en el diagnóstico <strong>de</strong> <strong>la</strong> arteritis <strong>de</strong> célu<strong>la</strong>s gigantes<br />

Respecto a <strong>la</strong>s p<strong>la</strong>quetas (> 400.000), tras aplicar el lrtest <strong>la</strong> significación fue<br />
p= < 0.05 .<br />
Ciento noventa y siete pacientes presentan según el algoritmo una probabilidad menor<br />
<strong>de</strong>l 10% <strong>de</strong> presentar una <strong>biopsia</strong> positiva. De éstos, 6 presentaron un resultado<br />
positivo (falsos negativos). Cuatro <strong>de</strong> estos 6 pacientes presentaban una clínica <strong>de</strong><br />
síntomas generales y polimialgia reumática sin sintomatología a nivel craneal. Los dos<br />
pacientes restantes <strong>de</strong>butaron con una clínica oftalmológica.<br />
Los pacientes que presentan más <strong>de</strong>l 90% <strong>de</strong> probabilida<strong>de</strong>s <strong>de</strong> tener una <strong>biopsia</strong> con<br />
resultado positivo fueron 17. De éstos, 16 presentaron una BAT positiva. El 100%<br />
fueron diagnosticados y tratados como una ACG.<br />
Los pacientes con una probabilidad <strong>de</strong> resultado <strong>de</strong> <strong>la</strong> <strong>biopsia</strong> mayor al 10% y menor<br />
<strong>de</strong>l 90% son en total 128. De éstos, 49 (38.28%) presentaron una <strong>biopsia</strong> <strong>de</strong> <strong>arteria</strong><br />
temporal positiva.<br />

Si <strong>de</strong>cidiéramos no realizar <strong>la</strong> BAT a <strong>los</strong> pacientes que presentan una probabilidad <strong>de</strong>l<br />
algoritmo menor <strong>de</strong>l 10% y mayor <strong>de</strong>l 90%, <strong>de</strong>jaríamos <strong>de</strong> realizar 214 BAT, el<br />
62,57% <strong>de</strong>l total <strong>de</strong> BAT realizadas. El coste sería <strong>de</strong>jar <strong>de</strong> diagnosticar a 6 pacientes.<br />
En 6 casos <strong>de</strong> <strong>los</strong> 197 pacientes <strong>la</strong> <strong>biopsia</strong> fue positiva. De el<strong>los</strong>, 4 pacientes<br />
presentaban síntomas generales sin clínica a nivel craneal. Los 2 pacientes restantes,<br />
<strong>de</strong>butaron con clínica oftalmológica.
