y vómitos. Solo una cuarta parte de los pacientes presenta sangrado rectal. Puede llegar a ser fatal si no se trata
precozmente. El enema con contraste hidrosoluble puede ser diagnóstico y terapéutico. Si no se reduce por este
medio hay que recurrir a la cirugía (19).
Apendicitis
Aproximadamente un 5% de pacientes pueden padecerla. La obstrucción del apéndice por impactación de moco
puede dar lugar a absceso apendicular y perforación del mismo. El tratamiento, como en cualquier apendicitis, es
quirúrgico (20).
Colonopatía fibrosante
En los diez últimos años, y al parecer relacionado con megadosis de enzimas pancreáticas con cubierta entérica, se
ha descrito la colonopatía fibrosante, que puede comenzar como una diarrea con hematoquecia de difícil control,
pero que evoluciona con engrosamiento de la pared del colon y posible estenosis colónica (21).
El estudio histopatológico revela fibrosis de mucosa y submucosa y criptitis focal. La aparición de esta patología
obligó a retirar la presentación de enzimas pancreáticos con altas dosis de lipasa y a recomendar que la dosis de
esta no sea superior a 6.000 U lipasa por kilogramo de peso corporal.
