Abstract Introduction: The right to free abortion until the twelfth week of pregnancy is widely accepted in the Danish population. The late induced abortions – abortions after the twelfth week of pregnancy – on the other hand, cause more disagreement and ambivalence. These are performed primarily because of fetal abnormalities or on social grounds. The introduction of prenatal diagnostics for all pregnant women in Denmark in 2004 has led to an increase in the number of late induced abortions performed because of fetal abnormalities. However, there is a lack of studies on how healthcare professionals experience working with the late induced abortions. Objective: To examine Danish midwives’ approach to late induced abortions, including how their professional identity, praxis and professional ethics develop in relation to this approach. Methods: Individual qualitative interviews with ten Danish midwives, all of whom had performed late induced abortions. The organisation and analysis of the data from the interviews is structuralised by the method of Grounded Theory as well as theories on professions that focus on how professional identity and professional ethics develop. Findings: All the midwives are of the opinion that performing late induced abortions is a part of their job. They all express their unreserved support to the woman’s autonomy, and as a result thoughts of the fetus’ right to live are suppressed. The general attitude of the midwives is that they have to remain professional when they handle late induced abortions, and therefore their personal opinions and any disagreements they might have, are suppressed. Yet, when directly asked, the midwives were skeptical about the termination of pregnancies because of nonlethal fetal abnormalities. We found that the performance of late induced abortions today resembles the birth of a living child because of the rituals that are performed, like showing the fetus to the woman, taking pictures of the fetus and making hand- and footprints. According to the midwives this is done to relieve the grief the parents might experience after the abortion. However, the benefits of performing these rituals, are not documented in the existing literature. Discussion: The everyday work of the midwives has changed and is limited by the institutional setting of the hospital, the technological development and the influence of other healthcare professionals. This challenges their belief in the normal birth and increases the demand for the midwives to legitimise their profession. In the defense of their profession
they tend to “over do” their job in relation to late induced abortions, and therefore it can be discussed whether the midwives themselves are creating the grief of the parents through the rituals they perform.
