<strong>ITALY</strong> - 6 Hazardous Waste<br />

C6.3.4.1. The manifest shall include the following information:<br />

C6.3.4.2. Name, address, and Unique Identification Number of the generator of the<br />

C6.3.4.3. Name, address, and financial code of the waste transporter<br />

C6.3.4.4. Origin, waste code, and quantity of waste<br />

C6.3.4.5. Disposal or recovery code to be used (see Appendices B.2 and B.3)<br />

C6.3.4.6. Name of disposal facility<br />

C6.3.4.7. Date and route to the disposal and/or treatment facility<br />

C6.3.4.8. Transport vehicle license plate number and driver’s name<br />

C6.3.4.9. Name and address of the receiver<br />

C6.3.4.10. The waste manifest shall be completed in quadruplet, all dated and signed by<br />

the waste generator and countersigned by the transporter. The waste generator shall retain one<br />

copy, and another copy must be returned by the transporter to the waste generator within three<br />

months of delivery of the waste to the disposal/treatment facility (or six months for out-ofcountry<br />

disposal). Installations shall notify the Italian Base Commander if the installation does<br />

not receive a copy of the manifest from the transporter within three months of the delivery of the<br />

waste for in-country disposal (or six months for out-of-country disposal).<br />

C6.3.4.11. Both copies of the waste manifest shall be kept on file for at least five years<br />

from the date of shipment. These copies shall be kept physically together with the waste loading<br />

and unloading register.<br />

C6.3.5. Record-Keeping Criteria<br />

C6.3.5.1. ID Number. Each generator will use the installation’s Unique Identification<br />

Number for all record-keeping, reports, and manifests for waste. For DoD internal tracking<br />

purposes, a separate unique identification number may also be used.<br />

C6.3.5.2. Each generator will use the USEUCOM Waste List to assign each waste a<br />

unique waste identification code. This unique waste identification code shall be used in all<br />

reporting, manifesting, and record-keeping.<br />

C6.3.5.3. Audit Trail. Generators will maintain an audit trail of HW from the point of<br />

generation to disposal. Generators using DLA Disposition Services will obtain a signed copy of<br />

the manifest from the initial DLA Disposition Services recipient of the waste, at which time<br />

DLA Disposition Services assumes responsibility. A generator, as provided in a host-tenant<br />

agreement, that uses the hazardous waste management and/or disposal program of a DoD<br />

September 2012 Italy FGS 6-10<br />
