ITALY - 8 Medical Waste Management
C8.3.4.2. The waste manifest shall be completed in quadruplet, all dated and signed by
the waste generator and countersigned by the transporter. The waste generator shall retain one
copy, and another copy must be returned by the transporter to the waste generator within three
months of delivery of the waste to the disposal/treatment facility (or six months for out-ofcountry
disposal). Installations shall notify the Italian Base Commander if the installation does
not receive a copy of the manifest from the transporter within three months of the delivery of the
waste for in-country disposal (or six months for out-of-country disposal).
C8.3.4.3. Both copies of the waste manifest shall be kept on file for at least five years
from the date of shipment. These copies shall be kept physically together with the waste loading
and unloading register.
C8.3.5. Record-Keeping Criteria
C8.3.5.1. ID Number. Each generator will use the installation’s Unique Identification
Number for all record-keeping, reports, and manifests for waste. For DoD internal tracking
purposes, a separate unique identification number may also be used.
C8.3.5.1.1. Each generator will use the USEUCOM Waste List to assign each waste
a unique waste identification code. This unique waste identification code shall be used in all
reporting, manifesting, and record-keeping.
C8.3.5.2. MUD Report (Annual Waste Report). An annual declaration of the waste
generated during the previous year shall be submitted to the Italian Base Commander (see
Chapter 1 for the process).
C8.3.5.3. Turn-In Documents. Turn-in documents [(e.g., waste manifests) and DD
Form 1348-1A] must be maintained for five years.
C8.3.5.4. Inspection Logs. Records of inspection logs shall be maintained for a period
of five years.
C8.3.5.5. Manifests. Manifests of incoming and outgoing special wastes, including
medical waste, will be retained for a period of five years. Two signed copies of the waste
manifest shall be retained together with the Loading and Unloading Register.
C8.3.5.6. Waste Analysis/Characterization Records. Records of waste
analysis/characterization shall be maintained for a period of five years.
C8.3.5.7. Medical Waste Treatment Records. Installations shall maintain records (for at
least five years after the date of disposal) of medical waste sterilization treatment, if any,
including date of treatment and records of the sequential identification numbers of sterilized
containers and of sterilization efficiency monitoring.
