<strong>ITALY</strong> - 6 Hazardous Waste<br />

component that has a different unique identification number (see definition), will obtain a signed<br />

copy of the manifest from the receiving component, at which time the receiving component will<br />

assume responsibility for subsequent storage, transfer, and disposal of the waste.<br />

C6.3.5.4. MUD Report (Annual Waste Report). An annual declaration of the waste<br />

generated during the previous year shall be submitted to the Italian Base Commander (see<br />

Chapter 1 for the process).<br />

C6.3.5.5. Turn-In Documents. Turn-in documents [(e.g., waste manifests) and DD<br />

Form 1348-1A] must be maintained for five years.<br />

C6.3.5.6. Hazardous Waste Log. A written log will be maintained for each TWSA<br />

and/or WSA. The log shall include the following:<br />

C6.3.5.6.1. Name/address of generator<br />

C6.3.5.6.2. Description and hazard class of the hazardous waste<br />

C6.3.5.6.3. Number and types of containers<br />

C6.3.5.6.4. Quantity of hazardous waste<br />

C6.3.5.6.5. Date stored<br />

C6.3.5.6.6. Storage location<br />

C6.3.5.6.7. Disposition data, to include: dates received, sealed, and transported<br />

and transporter used<br />

C6.3.5.7. Waste Loading and Unloading Register. In addition to the various waste logs<br />

to be maintained for each TWSA and/or WSA, a Waste Loading and Unloading Register shall be<br />

maintained at each installation, for both hazardous and non-hazardous waste. The waste register<br />

shall be used to record all manifested waste generated and shipped off base; entries shall be made<br />

within 10 business days from the generation/shipment operation. The waste loading and<br />

unloading register will be retained for five years after the date of its last entry, and will have the<br />

two signed copies of the waste manifest attached. More than one register can be present at each<br />

installation, as long as there are no duplicate entries.<br />

C6.3.5.8. Availability of Hazardous Waste Log. The Hazardous Waste Log will be<br />

available to emergency personnel in the event of a fire or spill. Logs will be maintained until<br />

closure of the installation.<br />

C6.3.5.9. Inspection Logs. Records of inspection logs shall be maintained for a period<br />

of five years.<br />

C6.3.5.10. Manifests. Manifests of incoming and outgoing special wastes, including<br />

medical waste, will be retained for a period of five years. Two signed copies of the waste<br />

manifest shall be retained together with the Loading and Unloading Register (see C6.3.4.7).<br />
