3.12 OCCURRENCE REPORTING<br />

The Occurrence Reporting program provides for the timely reporting <strong>of</strong> occurrences that<br />

could affect the safety <strong>of</strong> the public, seriously impact the intended purposes <strong>of</strong> the Site facilities,<br />

have an adverse affect on the environment, or endanger the health and safety <strong>of</strong> the workers<br />

Actual and potential TSR Violations or TSR Out-Of-Tolerance conditions are reported to<br />

cognizant management and to the DOE. Provisions <strong>of</strong> the program specify the processes for<br />

occurrence categorization, notification, investigation, root cause analysis, development <strong>of</strong><br />

corrective actions, tracking <strong>of</strong> corrective actions to completion, and lessons-learned<br />

determination.<br />

The Occurrence Reporting program is implemented by the Occurrence Reporting<br />

Process (Ref. 30) procedure.<br />

3.13 QUALITY ASSURANCE<br />

The Quality Assurance (QA) program assures a consistent and appropriate application <strong>of</strong><br />

quality requirements to the performance <strong>of</strong> activities using a graded approach. Safety,<br />

reliability, and performance are maximized through the application <strong>of</strong> effective management<br />

systems and graded controls commensurate with the risks posed by complex activities. Separate<br />

Quality Assurance Programs (QAPs) have been prepared that are similar in technical content,<br />

but differ in scope and applicability. One QAP is applicable to nuclear facilities activities with<br />

radiological’risks and is subject to DOE enforcement. The other QAP is applicable to non-<br />

nuclear’facilities and is a contractual obligation.<br />

QAP requirements are management systems, which are implemented through existing<br />

Site procedures and programs. The purpose <strong>of</strong> these management systems is to assist<br />

organizations in the accomplishment <strong>of</strong> mission objectives; to ensure work is planned and<br />

performed in accordance with regulatory and contractual requirements; and to ensure complex<br />

activities are conducted in an efficient and effective manner. QA is a shared, interdisciplinary<br />

function. It involves management and immediate worker contributions from all organizations<br />

responsible for performing activities to independently verify that activities comply with<br />

specified standards and requirements. The QAP establishes ten criteria associated with<br />

management, performance, and assessment: program, personnel training and qualification,<br />

quality improvement, documents and records, work processes, design, procurement, inspection<br />

and acceptance testing, management assessment, and independent assessment.<br />

An element <strong>of</strong> the QA program is the Management Assessment program. This program<br />

requires assessments be conducted to provide assurance to facility management <strong>of</strong> adequate<br />

implementation <strong>of</strong> SMPs and TSRs and to determine the effectiveness <strong>of</strong> those programs in<br />

support <strong>of</strong> facility operations. This program provides for early self-discovery and correction <strong>of</strong><br />

performance problems.<br />

A&rance Program Description (Ref. 3 1).<br />
