ACAMPROSATO CAMPRAL ® (Lab. Merck) • Caixas com 7 blisters de 12 comprimidos de 333 mg. pelo glutamato como os efeitos desagradáveis da abstinência 2 . O acamprosato parece ter ainda ação serotoninérgiga e beta-adrenérgica. O mecanismo neuroquímico pelo qual ele possui atividade anti-craving ainda não é completamente explicado 2 . FARMACOCINÉTICA E MODO DE USAR O acamprosato (acetil-homotaurinato de cálcio) é um psicotrópico utilizado no tratamento da dependência do álcool. É absorvido lentamente no trato gastrintestinal (menos de 10% do medicamento), e a administração conjunta de alimentos diminui a sua absorção. Sua meia-vida é de 1 a 3 horas, alcançando o estado de equilíbrio plasmático após 5 a 7 dias de uso contínuo, o que leva a uma concentração plasmática de 370 a 650 microgramas/L. Não se liga às proteínas plasmáticas. Sua excreção é predominantemente renal (90%), sendo contra-indicado em pacientes com insuficiência renal 1 . A administração do acamprosato pode ser iniciada logo após a desintoxicação do álcool, pois os níveis terapêuticos são atingidos em torno de 7 dias após o início de seu uso. A dose utilizada para o tratamento varia de 4 a 6 cápsulas ao dia. Sugere-se que sejam administradas da seguinte forma: 4 comprimidos ao dia (2 cp pela manhã, 1 cp à tarde e outro à noite) em pacientes com peso abaixo de 60 kg, e 6 comprimidos ao dia (2 cp pela manhã, à tarde e à noite) em pacientes com 60 kg ou mais.O acamprosato não deve ser administrado junto com as refeições, as quais reduziriam sua absorção 1 . FARMACODINÂMICA E MECANISMOS DE AÇÃO O acamprosato é um agonista gabaérgico, que apresenta estrutura química análoga à do GABA. Age inibindo a ação excitatória glutamatérgica no sistema nervoso central (SNC), atuando preferencialmente em receptores NMDA sobretudo quando estes se apresentam hiperativos 1 . Essa ação do acamprosato é compatível com a hipótese de que esse medicamento, suprimindo o aumento da atividade excitatória glutamatérgica no SNC, a qual ocorre em pacientes alcoolistas em abstinência, diminuiria tanto toxicidade induzida REAÇÕES ADVERSAS E EFEITOS COLATERAIS Mais comuns: diarréia e prurido. Menos comuns: dores abdominais, eritema maculopapular, náuseas e vômitos. INDICAÇÕES Evidências consistentes de eficácia: • alcoolismo 2-7 . CONTRA-INDICAÇÕES • Insuficiência renal; • insuficiência hepática grave; • hipersensibilidade ao medicamento. INTOXICAÇÃO Em casos de superdosagem, o sintoma mais comum é a diarréia, e o manejo indicado é a lavagem gástrica. Se houver sinais e sintomas de hipercalcemia, deve-se: • administrar por via endovenosa solução fisiológica a 0,9%; • aumentar a diurese com furosemida ou ácido etacrínico para acelerar a eliminação do cálcio; • monitorar a função cardíaca com ECG; • utilizar beta-bloqueadores se houver arritmias e, em casos mais graves, realizar hemodiálise. SITUAÇÕES ESPECIAIS Gravidez e lactação Esse medicamento está contra-indicado tanto na gravidez quanto na lactação. O acamprosato é eliminado no leite de animais de laboratório.
