Physician<br />

Agreement made between <strong>the</strong> <strong>City</strong> of <strong>Jersey</strong> city, a Municipal Corporation organized and existing<br />

under <strong>the</strong> laws of <strong>the</strong> State of New <strong>Jersey</strong>, with its principal place of business 280 Grove Street, in<br />

<strong>the</strong> <strong>City</strong> of <strong>Jersey</strong> <strong>City</strong>, <strong>the</strong> County of Hudson and <strong>the</strong> State of New <strong>Jersey</strong> (herein referred <strong>to</strong> as <strong>the</strong><br />

"<strong>City</strong>") and Solomon Owusu, M.D. (herein referred <strong>to</strong> as <strong>the</strong> "Physician") License number MA .<br />

Recitals<br />

i. <strong>City</strong> is a Municipal Corporation and in conduct of <strong>the</strong> Child Health Care Immunization<br />

Clinic under <strong>the</strong> auspices of <strong>the</strong> Department of Health and Human Services, Division of<br />

Health located at 20 i Cornelison A venue. The clinic desires <strong>to</strong> have <strong>the</strong> following services<br />

performed by <strong>the</strong> Physician.<br />

2.. Physician agrees <strong>to</strong> perform <strong>the</strong>se services for <strong>the</strong> <strong>City</strong> under <strong>the</strong> terms and conditions set<br />

forth in this contract.<br />

3. Physician is duly licensed <strong>to</strong> practice his/her profession in <strong>the</strong> State of<br />

i. NATURE OF WORK<br />

New <strong>Jersey</strong>.<br />

The physician will perform services with respect <strong>to</strong> all matters relating <strong>to</strong> or affecting <strong>the</strong> <strong>Jersey</strong><br />

<strong>City</strong> Child Health Care Immunization Clinic.<br />

· Physician shall provide medical services in accordance with <strong>the</strong> manual known as <strong>the</strong> Child<br />

Health Conference Manual, as published by <strong>the</strong> New <strong>Jersey</strong> State Department of Health, in<br />

accordance with Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program<br />

guidelines and Head Start Guidelines as established by <strong>the</strong> State of New <strong>Jersey</strong>.<br />

· a. Physician shall examine each child on admission, and as necessary, <strong>the</strong>reafter;<br />

b. On intervening visits, briefly evaluate <strong>the</strong> medical condition of <strong>the</strong> child;<br />

c. Evaluate and provide appropriate immunization and referrals<br />

d. Discuss health and emotional problems with parents and staff;<br />

e. Provide counseling in nutrition, feeding, growth and development areas;<br />

f. Provide medical documentation in <strong>the</strong> clinical setting;<br />

g. Administer immunizations and provide appropriate counseling.<br />
