AGREEMENT
THIS AGREEMENT entered into this _ day of , 2009 by and between the
City of Jersey City, (hereinafter referred to as the "City") and Educational Ars Team, Inc.
(hereinafter referred to as the "Subgrantee").
WITNESSETH THAT:
WHEREAS, THE Hudson County Deparment of Health and Human Services (hereinafter
referred to as the "County") awarded the City a grant in the amount of $241,739 to provide
educational and preventive substace abuse programs; and
WHREAS, in accordance with the grant requirements, the City solicited competitive proposals
for providing such programs; and
WHEREAS, in accordance with the terms and condition of the grant, the City is required to
utilize the services of the contractors and agencies identified in the City's grant application; and
WHREAS, the County approved the services described in the proposal submitted by the
Sub grantee to the City;
NOW, THEREFORE, in consideration of the mutul promises and other conditions, covenants,
and obligations made and agreed to by and between the parties, it is hereby agreed as follows:
1. Term of Agreement
Ths Agreement shall be effective as of the first day of Januar 1,2010 and terminate
on December 31,2010.
2. Work to be Performed
The Subgrantee shall, in satisfactory maner as determined by the City pedorm those
services outlined in the attched work plans and budget.
