II. TIME AND PLACE OF WORK
The Physician will be present as needed, and shall work hours agreed upon by the Physician and the
Coordinator of the Child Health Care Clinic of Jersey City, provided the Physician shall be available,
as necessary, for the entire period of a scheduled immunization session.
The Physician also will provide the foregoing service for immunization clinics, health related,
outreach programs for infants, pre-school children, adults and Child Health Conference sessions
suitabilityJor immunization of individuals and shall provide immunizations at community outreach
programs.
III. COMPENSATION
For all services rendered, under the terms of this agreement, the Physician is to be paid: $55.00 per
hour. The Physician shall bil the City for services rendered during the month, provided all elements
of the Agreement are satisfactorily met. The Physician will not bill the patient or health insurance
programs,
iv. RELATIONSHIP OF PARTIES
The parties agree that the Physician and the relation created by this contract is that of employer and
independent contractor. The Physician is not an employee of the City, is nQt entitled to the benefits
provided by the employer to its employees, including but not limited to group insurance, pension
plan and workmen and compensation benefits. Physician may practice his/her service for others
during those periods when the physician is not performing work under the contract for the City.
V. LIABILITY AND INSURANCE
The services to be rendered under this contract will be rendered entirely at the physician's risk. The
Physician shall purchase and maintain at his/her expense such comprehensive professional (Medical
Malpractice insurance for not less than $1,000,000) or other liability insurance coverage. The
Physician also agrees to defend, hold the City harmless and indemnify City for any lia~ilitl_9r loss
arising in any way out of the performance of the contract.
VI. FACILITIES
The City of Jersey City shall operate and maintain facilities and provide at its own cost supplies and
equipment necessary and proper to the performance of the Physician's position.
