3. Compliance with Approved Program
All activities authorized by this Agreement will be performed, as delineated in
Attachment A, in accordance with the approved work program, the approved budget,
the grant conditions and special conditions as required by the City.
4. Communications
The Subgrantee wil direct all communications concerning this Agreement to: Har
Melendez, Director, Deparment of Health and Human Services, 201 Cornelison
Avenue, Jersey City, New Jersey 07304. The Subgrantee has designated the
following as the individual to whom all communcations concernng this Agreement
will be directed to:

5. Report, Records and Evaluations
A) The Subgrantee agrees to submit to the City, in a timely maner, such reports
as may be required by the City, including the followig:
I) A quarerly program report shall be submitted to. the City by the tenth
workig day afer the end of each quarer.
II) A fiscal monitoring report and back up documentation shall be submitted on
a regular basis as required by the City. The format shall be determed by
the City.
B) The Sungrantee will make ávailable to the fiscal offcer of the City all
financial books, records, documents and files including payroll information as
well as any other material deemed necessar by the City fiscal offcer to
satisfy any audit requirements of this program.
