DETERMNATION OF VALUE CERTIFICATION
Har Melendez, of full age, hereby certifies as follows:
1. I am the Director of the City of Jersey City (City) Deparment of Health and Human Services
(DHHS) and have knowledge of
the goods and services provided by the DHHS.
2. The New Jersey City University - Peers Educating Peers (PEP) Program promotes drug-free
and non-violent values, attitudes and interpersonal skills through their substace abuse
prevention and education programs.
3. The City informally solicited quotations for services as professional services pursuant to
N.J.S.A. 40A:11-5(l)(a)(i).
4. The New Jersey City University Peers Educating Peers (PEP) Program submitted a proposal
to provide ongoing didactic presentations to a wide varety of communty agencies, with an
emphasis on connecting with several high schools, to provide mentorig and educational
programg that will focus on empowerig teens and helping them to develop coping skills
to address a varety of social issues for the sum of $42,000.00.
5. The DHHS recommendation is to award the contract to the New Jersey City University Peers
Educating Peers (PEP) Program.
6. The term of the contract is Januar 1,2010 though December 31, 2010.
7. The estimated alount ofthe contract exceeds $17,500.00.
8. This certification is made pursuant to N.J.S.A. 19:44A-20.5.
9. I certify that the foregoing statements are true. I am aware that if any of the foregoing
statements made by me are willfully false, I am subject to punshment.
