DETERMNATION OF VALUE CERTIFICATION
Hary Melendez, of full age, hereby certifies as follows:
1. lam the Director of the City of Jersey City (City) Deparment of Health and Human Services
(DHHS) and have knowledge of
the goods and services provided by the DHHS.
2. The Horizon Health Center - Communty.outreach through Peer Education (COPE) Program
promotes. drg-free and non-violent values, attitudes and interpersonal skils through their
substance abuse prevention and education programs.
3. The City inforially solicited quotations for services as professional services pursuant to
N.J.S.A. 40A:11-5(l)(a)(i).
4. The Horizon Health Center - Communty Outreach though Peer Education (COPE) Program
submitted a proposal to provide a minium of (300) adolescents and young adults residing in.
Jersey City with a vanety of opportties to develop coping skills with respect to problems
related to drug and alcohol abuse, while providing the means for them to remove the barers
that block their futue achievements in attainig self-suffciency for the sum of $42,000.00.
5. The DHHS recommendation is to award the contract to the Horizon Health Center-
Community Outreach thróugh Peer Education (COPE) Program.
6. The term of the contract is Janua 1,20010 though December 31,2010.
7. The estimated amount of the contract exceeds $17,500.00.
8. Ths certification is made pursuat to N.J.S.A. 19:44A-20.5.
9. I certify that the foregoing statements are true. I am aware that if any of the foregoing
statements made by me are willfully false, I am subject to punshment.
