Emphasize that although <strong>in</strong>sertion <strong>of</strong> an IUD is a m<strong>in</strong>or procedure, it is performed only<br />

by medical doctors (Ob-Gyn specialists) who are authorized and tra<strong>in</strong>ed to <strong>in</strong>sert an IUD.<br />

Although the mediator is never directly <strong>in</strong>volved <strong>in</strong> IUD <strong>in</strong>sertion, she should know about<br />

the process <strong>in</strong> order to educate women.<br />

Discuss with mediators the myths and rumors they have heard about IUDs and respond to<br />

any doubts participants may have.<br />

e) VOLUNTARY SURGICAL CONTRACEPTION (20 m<strong>in</strong>utes)<br />

Expla<strong>in</strong> that the other long-term methods are surgical contraceptive methods which are<br />

available on a voluntary basis to men and/or women who do not want any more children.<br />

Emphasize that these are permanent, irreversible methods and mediators should <strong>in</strong>form<br />

people very clearly about this po<strong>in</strong>t. They are safe and relatively free <strong>of</strong> side effects.<br />

Once the couple completes the size <strong>of</strong> their family, the man or woman may opt for<br />

permanent contraception.<br />

Expla<strong>in</strong> that there are two types <strong>of</strong> VSC:<br />

• Female Sterilization or Tubal Ligation for women<br />

• Male Sterilization or Vasectomy for men<br />

• Both are m<strong>in</strong>or operations, generally performed by <strong>in</strong>ject<strong>in</strong>g local anesthesia at the<br />

site <strong>of</strong> operation. Vasectomies are relatively simpler, safer, and less expensive than<br />

tubal ligation.<br />

Us<strong>in</strong>g the tra<strong>in</strong>er document Voluntary Surgical Contraception, briefly present the ma<strong>in</strong><br />

aspects <strong>of</strong> the two methods, for women and for men.<br />

Emphasize that:<br />

• Before choos<strong>in</strong>g this method, men and women must be <strong>in</strong>formed about the<br />

permanency <strong>of</strong> the method.<br />

• Persons must decide to undergo the surgical procedure voluntarily (i.e. without any<br />

force or coercion) after a complete understand<strong>in</strong>g <strong>of</strong> the relevant facts <strong>of</strong> the<br />

procedure. A consent form is used to document the <strong>in</strong>dividual’s agreement. Although<br />

there is no requirement for a spouse’s consent, a jo<strong>in</strong>t decision usually will mean a<br />

more satisfied client and fewer compla<strong>in</strong>ts to health providers follow<strong>in</strong>g the<br />

procedure. So both partners should be counseled about VSC.<br />

• Attempts to reverse vasectomy or tubal ligation are rarely successful. Therefore,<br />

couples who are consider<strong>in</strong>g VSC should be certa<strong>in</strong> that they do not wish to have any<br />

more children.<br />

Ask participants which persons can undergo VSC. Listen to their answers and then<br />

re<strong>in</strong>force the correct answer.<br />
