Relative Contribution of Cell Proliferation and Microvascularity to
Metastasis in Malignant Uveal Melanoma
Al-Jamal Rana›a (1) , Kivelä Tero
1. Helsinki University Central Hospital, Departement of Ophthalmology
Objective: To study the relationship between nucleolar size, proliferation,
extravascular matrix patterns, and dissemination of uveal melanoma (UM).
Method: A population-based, retrospective study in 167 consecutive patients
who had an eye enucleated because of UM from 1972-1981. Mean of the
ten largest nucleoli (MLN) was measured using silver staining. Proliferating
cells were identified using the antibody Ki-67. Insulin-like growth factor IGF-
1 receptor (IGF-1R) was used to evaluate tumorigenesis. A cross-sectional
analysis 37 metastases from corresponding primary UM were also analyzed.
Results: The association of large MLN with presence of epithelioid cells was
statistically significant. MLN was not associated with tumor diameter and
extravascular loops and networks. Ki-67 immunoreactivity, MLN, and
extravascular loops and networks independently predicted death, whereas
Ki-67 immunoreactivity was not an independent predictor. The percentage
of tumor area immunopositive for IGF-1R was not associated with survival.
Survival after diagnosis of metastasis was comparable whether hepatic
metastases had a large or small MLN, whereas a high MVD tended to be
associated with survival.
Conclusion: MLN and microvascular loops and networks were independent
predictors of survival. High cell proliferation index is independently associated
with risk of metastasis.
Prevalence of Depression Among Visually Impaired Patients and
Patients with a Dual Impairment in Belgium and the Netherlands
Van Rens Ger (1)
1. VUmc Amsterdam
GHMB van Rens, Vreeken HL, Nispen RMA van. The relationship between
visual impairment and depression is well known. In a recent study among
Dutch and Belgian patients we found that corrected for age the prevalence of
depression is even 2-6 times more in patients with dual impairment (hearing
loss and vision loss), compared to visual impairment only. Ophthalmologists
should be aware that the combination of dual impairment and a depression has
consequences for rehabilitation and quality of life of the patient.
Prevention of Depression in Patients with AMD by Early Visual
Rehabilitation - A Randomized and Controlled Pilot Study
Trauzettel-Klosinski Susanne (1)
1. Center for Ophthalmology, Low Vision Clinic, University of Tuebingen
Methods: 22 AMD-patients were randomly assigned to visual rehabilitation
including training and prescription of low vision aids at first presentation (T0)
(intervention group) or after a waiting period of 3 months (T1). Visual acuity,
reading speed, Geriatric Depression Scale (GDS), the German version of the
CES-D-Scale and National Eye Institute Visual Function Questionnaire (NEI-
VFQ) were assessed at first examination and after 3 months in both groups.
Results: Visual acuity diminished slightly in both groups whereas reading
speed increased significantly in the intervention group. Mean values of GDS
and CES-D-Scale diminished in the intervention group from (CES-D m at T0 =
8.8, m at T1 = 6.1; GDS m at T0 = 4.4, m at T1 = 4.0) whereas both variables
increased in the waiting group (CES-D m at T0 = 8.0, m at T1 = 10.9; GDS m
at T0 = 3.4, m at T1 = 4.2). Intervention group showed significant improvement
the sub-item “exercise of social roles” of NEI-VFQ (m at T0 = 31.6, m at T1 =
50.0, p = 0.03).
Conclusions: This study strongly indicates that professional visual rehabilitation
has positive effects on secondary depressive disorders in patients with AMD.
Psychiatric Approach in Management of Coexisting Vision
Impairment and Depression
Homer Robert (1)
1. Boca Raton Regional Hospital
Objective: Evaluation and effective psychiatric treatment for patients with
coexisting vision impairment and depression is addressed.
Method: The treatment team must determine if the patient is demonstrating an
appropriate reaction to their condition. If not, the severity of depression must
be assessed, and a determination made if referral to a specialist is appropriate.
In mild cases of depression, antidepressants, if indicated, might be prescribed
by the patient›s ophthalmologist or internist. The treatment team may also offer
supportive therapies to enhance coping and other skills. In more severe cases,
if diagnosis is uncertain, or if there is suicidal ideation, referral to a psychiatrist
or even hospitalization may be needed. Psychiatrists are routinely consulted
to treat depressed patients with coexisting medical disorders. Once under
psychiatric care, specialized therapies and psychopharmacological treatments
targeting individualized needs are utilized. Ideally psychiatric management is
also done in coordination with other treatment providers, psychologists, and
with family involvement.
Results: There are several psychiatric approaches available for the treatment
of depressed patients with medical stressors, though few studies specifically
address visually impaired patients.
Conclusions: Psychiatric approaches which include medication management,
offer support, improve coping skills, and offer realistic hope, are effective.
Impact of the Severity of Vision Impairment on Vision-related Quality
of Life in Germany
Finger Robert (1)
1. Center for Eye Research Australia
Assessing vision-related quality of life (VRQoL), and its relationship with the
severity of vision impairment and ocular conditions, even mild vision impairment
is independently associated with poor VRQoL. However a mild improvement
in vision following an intervention may not lead to a measurable difference
in reported VRQoL, even when employing scales allowing respondents to
weigh item importance in addition to conventional response categories. These
findings reinforce the importance of early preventative and rehabilitative efforts
to prevent deterioration in vision loss.
Topography and GP Lens Fitting
Aldelaigan Bassima (1)
1. BIN RUSHD Specialist Centre
Corneal topography is an essential tool when it comes to the diagnosis and
management of corneal abnormalities. The available topography technology,
the interpretation of different topographical maps and how to use them in the
fitting of contact lenses, especially fitting keratoconus patients will be reviewed
in this presentation.
Fitting Challenging Cases
Tanal Liliane (1)
1. Medicals International
Keratoconic and irregular corneas require a different fitting philosophy because
of their unusual corneal shape. Using a conventional GP lens design can
often result in discomfort, poor wearing time and ultimately, permanent
corneal damage. In each challenging fitting cases, a special type of contact
lens is needed. A practitioner should use a systematic approach when fitting
Keratoconus, irregular and post Surgical Corneas. However, fitting became
easier with the presence of the big range of Rose k family and with the newly
introduced hybrid contact lens, ‹synergeyes›.
