Relative Contribution <strong>of</strong> Cell Proliferation and Microvascularity to<br />
Metastasis in Malignant Uveal Melanoma<br />

Objective: To study the relationship between nucleolar size, proliferation,<br />
extravascular matrix patterns, and dissemination <strong>of</strong> uveal melanoma (UM).<br />
Method: A population-based, retrospective study in 167 consecutive patients<br />
who had an eye enucleated because <strong>of</strong> UM from 1972-1981. Mean <strong>of</strong> the<br />
ten largest nucleoli (MLN) was measured using silver staining. Proliferating<br />
cells were identified using the antibody Ki-67. Insulin-like growth factor IGF-<br />
1 receptor (IGF-1R) was used to evaluate tumorigenesis. A cross-sectional<br />
analysis 37 metastases from corresponding primary UM were also analyzed.<br />
Results: The association <strong>of</strong> large MLN with presence <strong>of</strong> epithelioid cells was<br />
statistically significant. MLN was not associated with tumor diameter and<br />
extravascular loops and networks. Ki-67 immunoreactivity, MLN, and<br />
extravascular loops and networks independently predicted death, whereas<br />
Ki-67 immunoreactivity was not an independent predictor. The percentage<br />
<strong>of</strong> tumor area immunopositive for IGF-1R was not associated with survival.<br />
Survival after diagnosis <strong>of</strong> metastasis was comparable whether hepatic<br />
metastases had a large or small MLN, whereas a high MVD tended to be<br />
associated with survival.<br />
Conclusion: MLN and microvascular loops and networks were independent<br />
predictors <strong>of</strong> survival. High cell proliferation index is independently associated<br />
with risk <strong>of</strong> metastasis.<br />

Prevalence <strong>of</strong> Depression Among Visually Impaired Patients and<br />
Patients with a Dual Impairment in Belgium and the Netherlands<br />

GHMB van Rens, Vreeken HL, Nispen RMA van. The relationship between<br />
visual impairment and depression is well known. In a recent study among<br />
Dutch and Belgian patients we found that corrected for age the prevalence <strong>of</strong><br />
depression is even 2-6 times more in patients with dual impairment (hearing<br />
loss and vision loss), compared to visual impairment only. Ophthalmologists<br />
should be aware that the combination <strong>of</strong> dual impairment and a depression has<br />
consequences for rehabilitation and quality <strong>of</strong> life <strong>of</strong> the patient.<br />

Prevention <strong>of</strong> Depression in Patients with AMD by Early Visual<br />
Rehabilitation - A Randomized and Controlled Pilot Study<br />

Methods: 22 AMD-patients were randomly assigned to visual rehabilitation<br />
including training and prescription <strong>of</strong> low vision aids at first presentation (T0)<br />
(intervention group) or after a waiting period <strong>of</strong> 3 months (T1). Visual acuity,<br />
reading speed, Geriatric Depression Scale (GDS), the German version <strong>of</strong> the<br />
CES-D-Scale and National Eye Institute Visual Function Questionnaire (NEI-<br />
VFQ) were assessed at first examination and after 3 months in both groups.<br />
Results: Visual acuity diminished slightly in both groups whereas reading<br />
speed increased significantly in the intervention group. Mean values <strong>of</strong> GDS<br />
and CES-D-Scale diminished in the intervention group from (CES-D m at T0 =<br />
8.8, m at T1 = 6.1; GDS m at T0 = 4.4, m at T1 = 4.0) whereas both variables<br />
increased in the waiting group (CES-D m at T0 = 8.0, m at T1 = 10.9; GDS m<br />
at T0 = 3.4, m at T1 = 4.2). Intervention group showed significant improvement<br />
the sub-item “exercise <strong>of</strong> social roles” <strong>of</strong> NEI-VFQ (m at T0 = 31.6, m at T1 =<br />
50.0, p = 0.03).<br />
Conclusions: This study strongly indicates that pr<strong>of</strong>essional visual rehabilitation<br />
has positive effects on secondary depressive disorders in patients with AMD.<br />

Psychiatric Approach in Management <strong>of</strong> Coexisting Vision<br />
Impairment and Depression<br />

Objective: Evaluation and effective psychiatric treatment for patients with<br />
coexisting vision impairment and depression is addressed.<br />
Method: The treatment team must determine if the patient is demonstrating an<br />
appropriate reaction to their condition. If not, the severity <strong>of</strong> depression must<br />
be assessed, and a determination made if referral to a specialist is appropriate.<br />
In mild cases <strong>of</strong> depression, antidepressants, if indicated, might be prescribed<br />
by the patient›s ophthalmologist or internist. The treatment team may also <strong>of</strong>fer<br />
supportive therapies to enhance coping and other skills. In more severe cases,<br />
if diagnosis is uncertain, or if there is suicidal ideation, referral to a psychiatrist<br />
or even hospitalization may be needed. Psychiatrists are routinely consulted<br />
to treat depressed patients with coexisting medical disorders. Once under<br />
psychiatric care, specialized therapies and psychopharmacological treatments<br />
targeting individualized needs are utilized. Ideally psychiatric management is<br />
also done in coordination with other treatment providers, psychologists, and<br />
with family involvement.<br />
Results: There are several psychiatric approaches available for the treatment<br />
<strong>of</strong> depressed patients with medical stressors, though few studies specifically<br />
address visually impaired patients.<br />
Conclusions: Psychiatric approaches which include medication management,<br />
<strong>of</strong>fer support, improve coping skills, and <strong>of</strong>fer realistic hope, are effective.<br />

Impact <strong>of</strong> the Severity <strong>of</strong> Vision Impairment on Vision-related Quality<br />
<strong>of</strong> Life in Germany<br />

Assessing vision-related quality <strong>of</strong> life (VRQoL), and its relationship with the<br />
severity <strong>of</strong> vision impairment and ocular conditions, even mild vision impairment<br />
is independently associated with poor VRQoL. However a mild improvement<br />
in vision following an intervention may not lead to a measurable difference<br />
in reported VRQoL, even when employing scales allowing respondents to<br />
weigh item importance in addition to conventional response categories. These<br />
findings reinforce the importance <strong>of</strong> early preventative and rehabilitative efforts<br />
to prevent deterioration in vision loss.<br />

in this presentation.<br />

Keratoconic and irregular corneas require a different fitting philosophy because<br />
<strong>of</strong> their unusual corneal shape. Using a conventional GP lens design can<br />
<strong>of</strong>ten result in discomfort, poor wearing time and ultimately, permanent<br />
corneal damage. In each challenging fitting cases, a special type <strong>of</strong> contact<br />
lens is needed. A practitioner should use a systematic approach when fitting<br />
Keratoconus, irregular and post Surgical Corneas. However, fitting became<br />
easier with the presence <strong>of</strong> the big range <strong>of</strong> Rose k family and with the newly<br />
introduced hybrid contact lens, ‹synergeyes›.<br />
